This exciting part time Admiral Nurse role has been developed in collaboration between Dementia UK and Berkshire Healthcare Foundation Trust for the East of Berkshire.
The Admiral Nurse will lead, deliver and co-ordinate relationship centered dementia care for younger people with dementia and their families in order to improve the experience of carers/families affected by young onset of dementia
Work base:
You will be based in Bracknell but will be working across the three East Berkshire localities, with access to bases in Maidenhead, Bracknell and Slough and regular opportunities to meet with the West Berkshire Admiral Nurse.
There is free staff parking at the main sites.
Main duties of the job
In line with the Admiral Nurse Competency framework at an enhanced specialist level the post holder will provide:
- Specialist bio-psychosocial assessment and support for people with young onset dementia and families with complex needs
- Facilitation of education and training for staff in the delivery of evidence-based young dementia care
- Clinical leadership with policy development and quality improvements in relation to young onset dementia care
- Consultancy to other health and social care professionals regarding young onset dementia
