An exciting opportunity has arisen to join the Acute Stroke service at Hampshire Hospitals NHS Foundation Trust. Based at the Royal Hampshire County Hospital, Winchester, the vacancy seeks to employ either a trainee Advanced Clinical Practitioner at band 7 with progression to an 8a, or a qualified ACP at band 8a level.

The ACP role is to work within the hyper acute emergency stroke pathway. Working alongside 2 current ACP's and providing clinical leadership for a team of 3 stroke practitioners the role aims to encompass working across the 4 pillars of advanced practice.

The role will cover a 7 day working pattern on a shift basis of long days, twilights (until 2200) and weekend working.

The HHFT stroke service receives approximately 2500 referrals to the acute bleep every year and has recently ,through much service development increased patient access to hyper acute treatments such as IV Thrombolysis, Mechanical Thrombectomy and Intracerebral Haemorrhage management. The role of the ACP is to manage patients who are referred to the stroke service autonomously or as part of a wider acute team alongside the medical team when patients present to the Emergency Department.

Main duties of the job

• Work within the 4 pillars of advanced practice as per job description and formal job plan.
• To provide expert assessment skills for patients presenting to the Emergency Department with a suspected acute stroke.
• Manage hyper acute stroke pathways and access to reperfusion interventions such as intravenous thrombolysis and mechanical thrombectomy.
• The ACP will practice both independently and in partnership with the multidisciplinary team under the clinical supervision delegated by a consultant and/or Specialist Registrar and/or under the guidance of the Lead ACP.
• Undertake advanced clinical assessment of the patient, analysing complex clinical signs, findings from physical examination, patient history, and investigation results to diagnose stroke/ non stroke medical/surgical conditions whilst considering differential diagnoses and initiating treatment.
• Interpret clinical results and act on findings, initiating treatments accordingly.
• Manages own autonomous caseload of patients through TIA clinic and work on a 1 in 3 weekend TIA rota for high risk patients.
• To take an active role in educating and developing members of the multidisciplinary team through formal and informal teaching sessions, including ED, ambulance service, medical teams.
• Maintains up to date knowledge acute stroke, using information to affect change in practice and ensuring the effective dissemination of new knowledge.
