Are you an experienced children's nurse? Would you like to work in a team enabling children with complex medical health needs in the community? If so come and joinour welcoming and supportive Children's Continuing Care and Assessment Team.

We support children with complex physical health needs in their homes, bycompleting Childrens Continuing Care assessment, providing their ongoing medical device and consumable supply, training, advocating and supporting the child or young person and their family enabling them to stay well and thrive in the community.

As part of the Children's Complex Needs Nursing Service we work closely with the wider children's nursing teams in LSCFT, and with the wider MDT in partner agencies supporting the child or young person's individual needs.

The team covers East Lancashire, Blackburn with Darwen, Preston and West Lancashire and this position will be based in our Pennine area, therefore travel across this area will be essential to this post on a regular basis. Our team operates Monday to Friday 8am until 5:30pm.

Main duties of the job

You will be NMC registered, and have post qualification children's nursingexperience including working with children with complex health needs in a multi-agency context. Experience working in a community setting and with children with tracheostomies and ventilation would be advantageous.

You will possess excellent clinical and communication skills and have knowledge and experience of children's complex needs nursing and the technology and equipment used by these children.

You will have great organisational skills and ability to use IT systems. The postrequires an ability to work independently accessing supervision as required. You will be highly motivated and dedicated to evidence based practice.

You will be responsible for children and young people with diverse physical healthneeds, undertaking assessments of children and young people in order to establish individualised management and treatment plans and determine appropriate interventions and goals. On a daily basis you will bring together safeguarding, clinical and case management skills to benefit the children and young people.

Qualifications

Essential

  • Professional Degree or equivalent
  • Registered nurse qualification - Child Branch or relevant experience
  • Current Professional Registration with NMC


Knowledge

Essential

  • Understanding of clinical governance framework & implications for practice, including audit and research governance
  • Knowledge of a broad range of assessment tools for children with diverse needs
  • Knowledge of a range of therapy techniques and programmes for children with diverse needs/ related to client group


Experience

Essential

  • Evidence of recent and relevant post graduate experience encompassing the specialist field of practice
  • Multi-disciplinary/multi-agency collaborative working
  • Experience of working with children with complex needs and their families in community/non clinical setting

  • Excellent interpersonal and communication skills both written and verbal
