Discharge Manager
We are looking to recruit a highly skilled professional (Nurse, Social Worker, OT) who will support the inpatient teams with allocating discharge placements for vulnerable service users. The successful candidate will need to identify appropriate placements for our service users, complete assessments and make referrals, and also to present funding packages to panels where needed.
The successful candidate will work with our local teams and private partners to ensure service users who access an acute bed are supported with a robust and supportive discharge package. The candidate will work with the inpatient teams to support plans for discharge, and also to support with sourcing and referring our service user for placements should it be required.
Main duties of the job
They must be able to work independently, be able to undertake comprehensive assessments and robust discharge plans, working collaboratively with service users, carers and clinical teams. Possession of comprehensive IT skills are essential as there will be a requirement to provide comprehensive performance data regarding clinical activity.
The successful candidate will be expected to work independently with the inpatient teams, and private partners to assist in the implementation of robust and supportive discharge packages. This could mean completing referrals, assessments, presenting cases for funding to panels, attending ward reviews and bed meetings.
