About the team:

The Carshalton and Wallington IRH provides excellent recovery focussed client care to the patients it supports. We now have a full-time opportunity for an experienced Band 6 CPN/Care Coordinator to work in the Carshalton and Wallington IRH. The successful applicant should be committed to high quality service provision with knowledge of psychosocial techniques, a desire to drive forward the social inclusion and recovery agendas, and understand and be committed to promoting and leadership, a high standard of patient care.

Main duties of the job

The Carshalton and Wallington IRH provides a specialist Borough based mental health support and recovery service for adults (18-75) with more complex and longer term needs which, because of their seriousness or complexity, cannot be effectively treated within primary care. Clinical/professional judgement will determine priority for treatment and support. The team works in close partnership with GPs and other key partners, including voluntary organisations. Care is delivered within the framework of the CPA. The team is committed to providing high quality mental health and social care for the local people of Carshalton and Wallington.

Job description Job responsibilities

About the role:

You will be part of the multi-disciplinary team, hold a designated caseload and be able to work independently. You will be expected to provide generic healthcare specific assessments. You must have good personal organisational skills, be willing to work flexibly and able to supervise students on placements and Recovery Support Workers. You should have demonstrable skills of working with people who experience serious mental health problems, experience of working in a multidisciplinary team and experience of assessing and managing crisis for people with a severe mental illness. A car driver/owner is essential.

We are based by the outskirts of the bustling town of Wallington at the Jubilee Health Centre.

Person Specification Education and Qualification Essential
  • Registered Adult Nurse and/or Registered Mental Health Nurse and/or Registered Learning Disabilities Nurse
  • Mentorship qualification
Desirable
  • Master's degree
Experience and Knowledge Essential
  • Experience of researchbased/reflective practice, e.g. understanding and ability to use supervision to reflect on skills, attitude and knowledge and develop these by using evidence based practice
  • Significant relevant clinical experience
  • Evidence of continuing professional development
  • Experience as a preceptor / mentor
  • Knowledge of clinical governance and audit
  • Knowledge of relevant Professional and Clinical legislation
Desirable
  • Experience of taking responsibility for a ward / clinical unit.
  • Experience of engaging / supporting clinical research projects
  • Experience of supporting practice development in clinical areas
  • Experience of teaching. e.g. lecturer practitioner/preceptorship
  • Experience of recruitment and selection of staff

  • Flexibility to work shift patterns across 24hrs, 7days a week and across all clinical areas as required
  • Ability to fulfil all the requirements of the role (including physical)
