Community mental health nurse (Older People's Mental Health)

Community health services ( Ealing, Hammersmith and Fulham, Hounslow)

We are seeking individuals who are passionate about working with over 70s residing in our socioeconomically and culturally diverse populations. This a new service with lots of opportunities for career development whilst working within supportive specialist multidisciplinary teams.

We encourage Band 5s who have completed their preceptorship and are eager for developmental opportunities to apply. We also welcome experienced Band 6 nurses

Our nurses will be responsible for a defined caseload of patients.

Main duties of the job

. Specialist assessment of individuals and their families

. Assessment and management of ongoing risks as identified during the assessment, ensuring that this is done in collaboration with service users and carers. Provide supervision of junior staff and trainees where appropriate.

. Work as a core member of an MDT.

. Act within the NMC Code of Conduct at all times.

. Deliver care under the Care Programme Approach and work in partnership with individual service users and their families and carers to ensure the delivery of the care plan.

. Administer and monitor medication, including depots, prescribed to individual service users, providing information and advice, within NMC guidelines and the Trusts Medicines Code.

. Broaden research and development skills through participation in local audit and research projects.

The role of Community Nurse is to work in partnership with service users and their carers in the Older People Community Mental Health Service with complex health presentations, developing their personalised care plans to enable them to lead fulfilling lives.

Person Specification Qualifications and Training Essential
  • Professionally qualified and registered Nurse wit current NMC registration in Mental Health / LD/ Adult Nursing.
  • Evidence of continuous professional development.
Desirable
  • Community Nursing qualification
  • Mentorship/teaching qualification.
Experience Essential
  • Experience of working with patients and their carers with complex health and social care difficulties in the designated care group.
  • Experience of supervising staff.
  • Experience of managing a caseload.
  • Experience of providing a range of clinical interventions to people in the designated care group with a variety of health difficulties.
  • Experience of working collaboratively with professionals.
  • Experience of conducting a range of clinical assessments including risk assessments.
  • Experience of partnership working within and across various statutory and non-
  • Previous experience of working with carers and independent / voluntary sectors.
Desirable
  • Experience working with older adults in receipt of healthcare.
  • Lived experience of mental health difficulties/caring for an older adult with mental health difficulties
  • Demonstrate a commitment to the provision of high quality and safe services with an ability to drive a culture of continuous improvement.
Knowledge Essential
  • Understands the principles of recovery / person centered care and their application to risk management, positive risk taking, and strengths orientated approaches to care delivery.
  • Have an excellent understanding of assessment and management of risk.
  • Have an understanding of the CPA process.
  • Have a good clinical understanding of the physiology and pharmacology in relation to older people's health.
  • Effective planning & organisational skills with an ability to prioritise own workload.
Desirable
  • Understanding of NICE guidance relevant to older people.
  • Have a good knowledge of service objectives
