A rare and exciting opportunity has arisen for a Band 6 physiotherapist to join the Integrated Independence Team in the Home Treatment and Reablement service.

The Integrated Independence Team (IIT) is a dynamic and committed service that provides multidisciplinary care to service users in Hackney. The IIT consists of many subteams and expertise across the teams, including Home Rehabilitation and Reablement, Rapid Response, Virtual Frailty Ward and Discharge to Assess.

The team consists of Physiotherapists, Occupational Therapists, Speech & Language Therapists, Social Workers and Assistants.

This post would suit an individual wishing to develop their clinical skills in community based rehabilitation and intermediate care, with an interest in frailty, falls and elderly care. You will be responsible for a varied caseload of clients with a range of health conditions. Our client group is clinically, economically and culturally diverse.

Main duties of the job

JOB SUMMARY
  • Provide specialist physiotherapy for a wide range of patients requiring assessment and rehabilitation in inpatient and community environments as part of a multi disciplinary team
  • Adapt physiotherapy practice to suit needs of patient, service and location of intervention
  • Provide clinical supervision and appraisal to junior staff, assistants and students dependent on rotation
  • Participate in ongoing service evaluation and development
  • Practice in a diverse cultural and socioeconomic locality
