We are looking for an experienced, motivated and enthusiastic team player to join our mature, friendly, dynamic progressive heart failure and cardiac Rehabilitation Team covering the Princess Royal University Hospital and Bromley Community.
Main duties of the job
This role will involve identifying, supporting and delivering high quality heart failure as well as cardiac rehabilitation in line with the commissioning pack to patients following Myocardial Infarction, arrhythmias Cardiac Surgery, coronary angioplasty /AICD, Heart Failure and a range of other cardiac conditions and procedures.You will be expected to engage in the facilitation of the Heart Manual, co-ordinate and take part in the outpatient exercise and information programme (based at the Princess Royal University Hospital Beckenham Spa and any other identified south London site), clinical consultations and be responsible for data collection using the NACR database for cardiac rehabilitation and NICOR for heart failure. Working and communicating well within a small team and across acute and primary care regarding cardiac rehabilitation and heart failure will be essential.

- To work as an autonomous advanced practitioner and effectively deliver an expert nursing service to patients with a diagnosis of heart failure and be able to optimize heart failure patients prognostic medication while they are undertaking their cardiac rehabilitation program in line with National standards.
- An independent practitioner with skills and knowledge to assess and deliver care to patients and carers following a cardiac event and heart failure.
- To ensure that patients and their families are fully informed and supported by providing clinical andsocial support
- To assist in the development of evidence based clinical nursing practice in line with current research and guidelines
- To establish clear lines of communication within the heart failure, cardiac rehabilitation and other cardiac subspecialties to ensure cohesive multidisciplinary management of patients.
- To link with healthcare teams to increase knowledge and facilitate the care of this group of people in the community.
- To be the liaison between the primary and secondary care for the patient.
- To link with the tertiary centre on a regular basis to discuss patients and seek role development andsupervision
- Articulates clear evidence-based information to patients in the community and hospital setting and over the telephone
- Participate in pre-assessment in inpatient and in outpatient clinics and co-ordinates and participatesin the out-patient exercise programmes

- To assist in the delivery of an advanced specialist nursing service for people with a diagnosis of heart failure.
- To provide specialist nursing advice and support to patients, their families and other healthcareprofessionals following diagnosis and through treatment.
- To ensure continuity of a high standard of evidence based nursing care, assessing health, healthrelated and nursing needs of patients, their families and other carers by identifying and initiating appropriate steps for effective care. This can include:
- Managing a patient caseload
- Ordering and evaluating diagnostic tests
- Making and receiving referrals
- Admitting and discharging patients for specified conditions and within agreed protocols
- Running own clinics (both community based, and hospital based), using advanced clinical skills to assess patient conditions and make treatment decisions
- Clinically assessing, prescribing & monitoring the efficacy of appropriate drug therapies
- Patients in the outpatient exercise setting are always safe current ALS/ ILS certification andupdates organised in line with the Trust and national policies.
- To accept responsibility for own patient caseload, ensuring all patients have an accurate plan of care,which reflects the assessment undertaken and incorporates the issues and recommendations madeensuring clear documentation in the patients records and handheld records
- To follow-up patients as appropriate and ensure all relevant information is available to enable patientsto make an informed choice about their treatment
- To attend and co-ordinate MDT meetings ensuring all patients are represented
- To deliver a seamless service through the development of enhanced Multidisciplinary team (MDT) processes and communication
