INTERNAL ONLY TO NORTH CUMBRIA INTEGRATED CARE NHS FOUNDATION TRUST (NCIC) TRUST EMPLOYEES ONLY
To undertake a development leader role across the ICCs and community collaborative, working closely with GPs, AHPs, nurses, ASC, reablement, voluntary agencies, primary care and mental health services, North Cumbria Integrated Care NHS Foundation Trust (NCIC) acute and community hospitals and other department Leads.
To work as a senior member of Home Care Practitioner service to co-ordinate, deliver, develop and evaluate the care provided to patients on a day to day basis in close liaison with the other team members working within this service and the wider community and ICC team.
To support patient flow through the health and care system and prevent admission where safe to do so.
To work as a senior member of ICCs to support the delivery of the national and local discharge requirements.
The role will have an emphasis on leading service improvement, and facilitating quality activity in close collaboration with other team members. The role will support current quality initiatives including training, outcome measures, service evaluations, audit and evidence-based practice including performance measures.

Main duties of the job
- To be professionally and legally responsible and accountable for all aspects of your own work including the management of patients in your care. To ensure a high standard of care for the patients under your management, and support more junior staff to do likewise
- To support the comprehensive assessment of patients, including those with a complex presentation, using investigative and analytical skills, and to formulate individualised management and treatment plans.
- To accept professional responsibility for a designated caseload of patients, and to organise this effectively and efficiently with regard to clinical priorities and use of time. To take responsibility for prioritising patients referred to your speciality and to delegate care of patients appropriately to more junior staff.
- To ensure that designated staff implement policy and service development changes.
- To provide spontaneous and planned advice, teaching and instruction to relatives, carers and other professionals, to promote understanding of the aims of interventions and to ensure a consistent approach to patient care.
