The Trauma Patient Flow Co-ordinator will be responsible for effective utilisation of beds and patient flow both unscheduled and scheduled admissions.

Working in conjunction with the Emergency Department, admission/assessment portals, wards and alongside other members of the Capacity management Team, ensuring national patient access standards are met within a quality framework.

Main duties of the job

To facilitate the effective use of beds, using a structured approach and working alongside clinicians, nurses and the Site Matron, to ensure pro-active appropriate management and use of UHNM bed capacity. Being instrumental in ensuring patient access targets are met.

Ensure pro-active planning to allow accommodation of both elective and emergency patientsAssist Clinicians and Senior Nursing staff with the management of all admissions and discharges for the division's patients by facilitating an efficient, effective and timely utilisation of beds and resources.Proactively challenge patient flow within the divisionProvide support and organise resources for the expedition of discharges across the division, ensuring patients are expedited to the discharge lounge within appropriate criteria.Liaise with neighbouring hospitals to arrange patient transfers to and from the DivisionsData analysis of patient discharge and admission trends when requiredTroubleshoot and identify areas of improvement with regards to patient flow, working closely with the Ward Managers and Matrons within the divisionChair and manage the divisional capacity meetings.

Job responsibilities

  • Liaise closely and regularly with ward nursing staff and Patient Flow Co-ordinator to ensure accurate up to date knowledge of patient movement is obtained.
  • Liaise as appropriate with the discharge Co-ordinators and multi-disciplinary team with regard to discharge issues. Identify problems affecting the efficiency and effectiveness of admitting and discharging patients, making these known to the relevant managersDay to day line management of flow coordinators in accordance with trust policy whilst on shiftMaintain accurate reporting arrangements e.g. through a database, regarding the level of emergency demand, patient outlier numbers and occupancy to improve capacity/demand managementParticipate in appropriate Trust wide as appropriateTo liaise closely with the Site Matron and senior nurses if staffing issues are affecting bed capacityTo participate fully in any project work involving bed management to ensure its successKeep up to date of all new local and national emergency care targets and Clinical IndicatorsWork closely with the Site Matron to ensure the right patient is in the right place at the right time
  • Provide live next day discharge data alongside the patient flow coordinators to promote early expedition of dischargesCheck, confirm and challenge estimated dates of discharge on the WardsEnsure complete utilisation of the discharge lounge or equivalent
  • Ensure that Ward Teams advocate a 7 day discharge ethos and that management plans are in place for effective weekend discharge of patients


Person Specification

Experience

Essential

  • Experience of patient flow in an acute setting


Desirable

  • Experience of patient flow within the trauma speciality
  • Knowledge of IT systems


Qualifications

Essential

  • GCSE or equivalent in maths and english


Desirable

  • Health or operational related further education qualification - A levels, BTEC


Knowledge

Essential

  • understanding of trauma and major trauma patient requirements


Desirable

  • Understanding of major trauma and trauma patient pathways in acute major trauma centresetting
