Join our Community Integrated Team, a multidisciplinary team committed to providing exceptional nursing and therapeutic care. Our service operates between 0800 - 1800, 7 days a week, supporting patients in their own homes with the goal of promoting maximum independence through self-management in daily activities.

You will need to be an effective team player with the ability to work independently with a strong commitment to collaborative working.

You will have the ability to support operational managers in resourcing the service and an understanding of professional and clinical decisions that influence procurement.

Main duties of the job

As a Registered Adult Nurse in our team, you will:

Case Management: Manage care for a designated number of patients, working closely with GPs, Integrated Care Teams (ICTs), and other statutory and voluntary agencies, with a particular focus on diabetic patients.

Patient Education: Provide advice, information, and education to patients, families, and carers, enabling patients to achieve maximum independence, health, and well-being.

Self-Care Promotion: Encourage self-care management to prevent the worsening of conditions and improve health outcomes.

Best Practice: Ensure delivery of best practices in clinical care, caseload management, and education for both patients and team members.

Specialist Resource: Act as a specialist resource, offering expert advice on diabetes to patients, their families/carers, and health professionals.

Service Improvement: Enhance diabetes services for housebound and care home patients, including liaising with secondary care diabetes services.

Leadership in Projects: Lead the implementation, education, and training of the insulin care home project, supporting care homes in administering insulin to their residents.

  • oEvidence of recent CPD
  • oRGN/RN ADULT, with current NMC Registration
  • oDiploma in a diabetes related field or equivalent post graduate qualification.

  • oMentorship qualification

  • oNon-Medical Independent Prescriber


Experience

  • oProven post qualification experience
  • oExperience of working with professionals from other disciplines
  • oExperience of working as a member of a team but also able to work autonomously
