An exciting opportunity has arisen within the MHSOP Community Mental Health Team to operate as a Community Mental Health Nurse within the Older Adult's pathway within a specific Geographical area.

To be professionally accountable and responsible for service user care. To undertake a range of clinical / therapeutic interventions within the service user's home environment or other community settings.

Main duties of the job

Working as a community mental health nurse within Mental Health Services for Older People, your role will be to undertake the assessment and management of services users presenting with complex mental health care needs.

You will provide a comprehensive assessment of need, offering intervention, advice and support to service users, family, carers and other professionals involved in their care.

As part of the multidisciplinary team, you will ensure that care and treatment is delivered to a I high quality, ensuring that person centred care is deliver, risk are managed and appropriate treatment and other interventions (pharmacological and non-pharmacological) are offered within I timely manner.

You will contribute to the effective care planning of the service user's needs by providing regular reviews, risk assessment with the goal of working towards safe discharge planning.

Person Specification

Qualifications

Essential

• Nurse on Part 3 or 13 of the NMC Professional Register
• ENB 998/City and Guilds 730 or Facilitated learning and assessment in practice qualification
• Mentorship qualification
• RMN essential

Desirable

• Post basic qualification at Diploma / Degree level or evidence of such a qualification being worked towards

Experience

Essential

• Significant post registration experience in Elderly Mental Health Care
• Evidence of qualification to undertake the supervision, teaching and assessing of students (essential requirement for post, expectation this will be completed within an 18 month period).
