Dorset HealthCare are currently seeking a Community Staff Nurse to join our vibrant and innovative Integrated Care Team in Purbeck, in the District and Community Nurse team in Wareham.
You would work alongside our experienced Community Nurse Teams helping to provide care needs to a high-quality standard of care for patients in their own homes.
We can be flexible with shift patterns as long as the service needs are met between hours of 08:00-20:00 with some attractive weekend enhanced pay.
We offer a structured induction programme, flexible working patterns and pastoral support. There are excellent learning and development opportunities, both in-house and through our links with local universities, plus the option of a preceptorship programme for newly qualified nurses to boost your skill development.
Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.
Main duties of the job
Main duties are to provide high-quality person-centred approach to care delivery which always considers people's safety, privacy, dignity whilst promoting and embodying the Trust's values. Undertake assessment of patients, care needs, and develop programmes of care as part of the wider multi-disciplinary care team.
As a Community Staff Nurse, you would manage acute and chronic nursing needs, be able to develop strong, therapeutic relationships with your patients and work closely with community partners to achieve the best possible health outcomes.
You would be identifying care needs, monitoring patients' conditions, recording observations and helping them develop their wellbeing care plans.
You'd be supporting people to maintain their wellbeing and independence, helping to avoid unnecessary admission to hospital as well as delivering exceptional end of life care including symptom management and help in identifying and achieving people's preferences.
