  • We are seeking a skilled and experienced nurse specialising in neurology, to join our well established and respected, multi-disciplinary Community Neuro Team within the borough of Wandsworth.
  • The successful applicant will be responsible for the assessment and treatment of adults and older people with a variety of neurological conditions including Early Supported Discharge (ESD) for stroke patients.
  • Specialist knowledge and experience of neurological medicines and continence management would be advantageous.
  • There is a strong emphasis on multi-disciplinary working especially within service development and team learning.
  • The MD team also includes occupational therapists, physiotherapists, speech and language therapists, dietician, clinical neuropsychologists, neuronavigator, Therapy Assistant Practitioners, admin and psychology assistants.
  • The post holder will have a key role in the overall community neuro rehab service development, and also a lead role within wider neuro nursing pathways in Wandsworth (for example continence, stroke, MND and end of life care) and St George's Hospital services.
  • Continuing professional development is actively supported via clinical supervision, annual Personal Development Plans, post-graduate and in-service training from a uni-professional and multi-disciplinary point of view. Supervision of students is very much encouraged.
  • All our staff are fully supported by the Clinical Team Leader for the Community Neuro Team.


Main duties of the job

  • To work as an autonomous, advanced practitioner and deliver an expert nursing service within the specialty of community neuro rehabilitation.
  • Provide effective hands on nursing care, support, advice, supervision and education to clients, carers and relatives with complex neurological needs.
  • To ensure that patients and their families are fully informed and supported by providing excellent clinical knowledge and support.
  • To assist in the development of evidence based clinical nursing practice in line with current research and guidelines.
  • To establish clear lines of communication within the specialty pathways to ensure cohesive multidisciplinary management of patients.
  • To link with other healthcare teams in order to increase knowledge and facilitate the care of this group of people in all healthcare settings.

Person Specification

Qualifications

Essential

  • NMC Level 1 or Level 15 Registered Nurse
  • Degree (BSc) or equivalent in a health related subject, or working towards relevant post-registration qualification in specialist field


Desirable

  • Supplementary/Independent Nurse Prescribing qualification
  • Counselling qualification


Experience

Essential

  • An advanced practitioner either through formal qualification (as above) and/or extensive clinical experience in neurology/neurosurgery (acute, inpatient or community)
  • Extensive relevant, demonstrable experience in specialty area (neurology/neurosurgery/stroke)
  • Facilitating meetings and groups
  • Ability to impart complex information to patients, relatives and carers, with different levels of understanding
  • Giving and receiving of clinical supervision and appraisals
  • Experience of delivering teaching sessions and/or presentations


Desirable

  • Experience of running research and audit programmes
  • To have proven experience of service development


Skills

Essential

Knowledge

Essential

  • Up to date knowledge of current nursing clinical guidelines relevant for the delivery of a community neuro service
  • To have a good understanding (and experience) of managing continence for patients with a neurological condition
  • To have a good understanding of commonly used medication in neurological and urological conditions.
  • Understanding of clinical governance and risk assessment
  • Clear understanding of safeguarding procedures (adult and child)
  • Computer literacy
