We are looking for Registered Mental Health Nurses/ Mental Health Social Workers to join Single Point of Access/Emotional Wellbeing Service , a developing front line service. Our team prides itself on being a group of experienced, supportive and motivated individuals.

We are a team who sees, and very much values potential. With this in mind, we also have specially designed Band 5 progression posts. This is offered to candidates who we feel would benefit from further support to attain a Band 6 level

There is scope to influence and improve the experience and pathways for people who need to access adult mental health services.

The post is suitable for Registered Mental health Nurses and Social Workers with at least 18 months experience of working in Mental Health Services .

Skills in assessing, planning, evaluating and managing care are essential, as is a working knowledge of social care management.

Knowledge and understanding of PDR and supervision processes are also essential requirements for the post together with a commitment to the appraiser and supervisor role.

Additionally, you will need to possess excellent communication skills, be motivated, able to work independently and as part of a multidisciplinary team, and able to work with other partnership agencies.

You should have a positive approach to meeting the needs of service users and be able to demonstrate continued professional development. There will be further opportunities for training and development.

Main duties of the job

The Single Point of Access/Emotional Wellbeing Service (SPA/EWS) is a multi-disciplinary team which provides a service to people who are between 18 and 65 years of age, registered with a GP surgery in Sheffield.

SPA/EWS provides a first point of contact for anybody wishing to access Mental Health services in Sheffield.

The objective of the service is to streamline all referrals to mental health services. The team provides an in-depth triage and assessment process. Following assessment there is the potential to offer short term mental health interventions to treat and/or signpost the service user to the most appropriate mental health pathway.

The service operates between the hours of 8:00am - 8:00pm Monday to Friday and works across two locations. The SPA triaging component is based within the Northern General Hospital site as part of the Crisis Hub. The EWS assessment and short term work component is based at Netherthorpe House. Post holders will be expected to rotate across the two sites.

The service links with other teams within SHSC and partner organisations across the public and private sectors to facilitate access to appropriate care, treatment and support.

Job Purpose

To triage and process all Routine/urgent and crisis referrals to the Adult Mental Health Services through Single Point of Access team.

To carry out comprehensive mental health assessments and respond to crisis/urgent referrals. To ensure that an evidence based, and focussed plan of care is formulated following assessments.

Where appropriate, to provide short term focussed interventions or signpost the service user to other appropriate care Pathways.

To work closely alongside the Out of Hours Team, the Home Treatment Team, the AMHP service and partner organisations across the public and private sector to facilitate access to appropriate care, treatment and support.

General

Maintain Professional registration and adhere to professional guidelines and code of conduct.

Work within the framework of relevant policy and legislation both local and national

Job Summary

The post holder will assist in the processing of referrals to the SPA/EWS team within a timely manner through contribution to the SPA function of the Team.

The Post holder will carry out routine/urgent and crisis assessments.

The Post holder will manage a defined caseload and carry out focussed time limited interventions.

Will be responsible for ensuring that effective communication pathways exist between the service user and other key professionals.

Will be responsible for identifying training needs necessary to ensure provision of evidenced based care as appropriate.

Will contribute to service development within SPA/EWS.

Supervision

Regularly review own caseload with supervisor to promote efficacy and contribute to the overall management of locality CMHT.

Deliver supervision and facilitate annual PDR with more junior members of staff.

Participate in own PDR process, identifying training and development needs, and pursuing relevant actions to maintain and improve level of competence and effective contribution.

Service Development

Act as mentor to learners, keep up to date with relevant course and curriculum developments, and maintain sign off status.

Participate in training relevant to role.

Promote insight and understanding of mental health issues to service users, carers, and other agencies.

Contribute to audit, research and evaluation as appropriate.

Actively engage in team initiatives to develop the service.

Person Specification Qualifications Essential
  • RMHN/Allied Health Professionals having current registration with the relevant regulatory body
  • Evidence of continuous professional development within the field of mental health
Desirable
  • Teaching/Assessing qualification or equivalent.
Experience Essential
  • Currently working as a Band 6 RMHN or equivalent professional post.
  • If not current Band 6 (or equivalent) at least 12 months experience following preceptorship
  • Experience of working in acute mental health care setting
Desirable
  • Experience of working within a Community Mental health setting
  • able to work flexibly between the hours of 8am and 8pm
  • to have experience of working within crisis and acute care
  • to be a skilled assessor
