We are seeking a Band 6 Nurse to join the Paediatric Hospital at Home Team and work alongside the team continuing the support and development of the Hospital at Home Service further. The post holder will need to be enthusiastic, patient focused, flexible and able to work independently within a busy multi-professional team.
The service runs 08.00 - 20.00 Monday to Friday and 08:00-18:00 on Weekends, 7 days a week, 52 weeks of the year, and requires some flexibility across working patterns.
The service takes referrals for the following minor illnesses:
Main duties of the job
To be an active member of the Hospital at Home team preventing unnecessary admission to, or facilitating discharge from inpatient areas. To work with colleagues in Primary and Secondary care to support the management of sub acutely ill patients in their own homes when appropriate and safe to do so.
When the nurse visits the child at home, they will carry out an assessment, which includes recording a pulse rate, respiratory rate, oxygen saturation level, temperature and blood pressure plotting them recordings on a paediatric early warning score chart for the appropriate age of the child.
The team also assist the family to care for their child, the team will help educate parents on managing this illness and future illnesses that the child might be susceptible to. The team use current up to date evidenced based practice and utilise safety netting advice supported by Healthier Together.
The team also signpost to healthcare provisions and which health care services to access depending on the clinical need of the patient.

Job responsibilities
The Hospital at Home service is a trust wide services that aims to provide clinical observations and support for families in their own homes.
The job would entail taking referrals from sources such as inpatient ward or assessment area, local GP services and our emergency department. The nurse on duty will complete a referral form, screening the patients against our referral criteria. If the patient meets criteria the team aim to contact the family via telephone within 2 hours and where possible, around other visits and lone working policies, will attend the home within 4 hours.
Where the referral does not meet criteria but the patient requires further assessment the nurse on duty will signpost to the relevant service, for example to the Paediatric Medical team on call or to the emergency department.
The post holder should have clinical expertise and knowledge in paediatric care and assessment, being able to competently manage and support a variety of childhood illnesses.
The post holder should work to their limitations and escalate concerns to the relevant team if the patient deteriorates or shows signs of clinical concern.
The post holder will work as part of a team of band 6 nurses who are responsible to the Hospital at Home Band 7 Lead and the DPoW Paediatric and Neonatal Manager. The post holder should raise any concerns to either management team in regards to any of the staff members health and/or performance.
The post holder should adhere to national, regional and local trust policies and procedures, ensuring they are working within evidenced based practice.
The post holder should have a good understanding of Children's Safeguarding and be able to identify and manage caseloads with safeguarding concerns. Documenting these conversations and visits on System One and inline with NMC standards.

- Viral induced wheeze/asthma
- Gastroenteritis/diarrhoea and vomiting
- Fever of known origin
