An exciting opportunity has arisen to join a new, innovative and dynamic team within our expanding Frailty Virtual Ward service. We are looking for experienced and motivated senior nurse (Band 7) who is passionate about service development and providing high standard of patient centered care. You will be working closely within a Multi-Disciplinary Team of staff including Doctors, senior nurse practitioners, Occupational Therapists, Physiotherapists and HCA's, all of whom are committed to maintaining independence, promoting self-care and assisting patients in their usual place of residence. At times this post may require you to work autonomously, when working across the Hackney borough into patients own homes and in an inpatient hospital setting.

Main duties of the job

o The post holder will practice within a multidisciplinary team and provide care topatients, assessing, planning, implementing and evaluating their care needs in the hospital and patients own homes.o Working within the Frailty Virtual Ward to prevent unnecessary admissions and/or facilitate timely discharge from hospital/ other healthcare settings under direct or indirect supervision of senior medical colleagues.o Responsible for delivering a high standard of holistic nursing care whilstpromoting and maintaining health education and independence.o Participate in service development within Frailty Virtual Ward service set up, audits and outcome measures to embed a culture of continuous improvement and positive outcomes for patients.o Work in a challenging socioeconomic environment involving patients who have highly complex social problems and those from culturally diverse backgrounds.o Deputise for Band 8A Frailty Virtual Ward Lead in their absence takingresponsibility for operationally managing allocated parts of the service.

Education/Qualifications

Essential

  • Registered General Nurse (NMC Registered)
  • Evidence of ongoing post registration training.


Desirable

  • Advanced Physical Assessment Course or willingness to undertake
  • Educated to master's level or equivalent experience
  • Non Medical Prescribing (willingness to undertake)


Skills/ Abilities

Essential

  • Experience managing the complex needs of patients in a variety of settings
  • Phlebotomy skills
  • Cannulation
  • Able to identify a deteriorating patient (Sepsis, mental health crisis, selfneglect, end of life)
  • Ability to assess and treat variety of wounds (surgical, traumatic, pressure ulcer, leg ulcer, moisture lesions)
  • Able to work using your own initiative.


Experience

Essential

  • Post registration experience as a band 6
  • Experience of identifying, reporting, managing and escalating safeguarding concerns
  • Experience of undertaking first contact/assessments in the community


Knowledge

Essential

  • Understanding Mental Capacity Act 2005 and able to complete MCAs, best interest decision making
  • Understanding of clinical governance and ensuring team compliance
  • Understanding of professional accountability and delegation issues
  • Understanding of professional accountability issues
