Stamford Unit Senior Sister/Charge Nurse - Band 7 37.5 Hours per week

Stamford Unit is a 96 bedded intermediate care unit consisting of 3 floors or 32 beds each. We provide care for patients through four pathways rehabilitation, complex discharge planning, community step up and end of life.

SECTION 1- MAIN RESPONSIBILITIES:
PROFESSIONAL/CLINICAL:
1 Ensure the unsupervised delivery of evidence based nursing care by providing expert clinical assessment, care planning, care delivery, care evaluation and clinical advice.
2 To be professionally and legally responsible and accountable for all aspects of your own work, including management and responsibility for more junior members of staff and all patients and working as an autonomous clinical practitioner. Actively demonstrate expertise to all levels of staff, including new medical staff. To respond to frequent request and interruptions from all grades of staff and from medical staff, with regard to patient care problems and the organisation of the unit, patient management and bed management.
3 Working in accordance with the NMC Code of Professional Conduct, competently undertake expanded roles, and teach and assess the competence of other clinical staff to do so, to enhance the delivery of patient care.
Using own judgement, initiate appropriate interventions prior to medical input being available, including those with complex presentation using analytical skills.
4 Provide spontaneous advise, teaching and instruction to other grades of staff and professionals including new members of medical staff.
5 Maintain accurate and contemporaneous records and ensure that all records relating to patient care is clear, concise and legible in accordance with the Trust and NMC/HPCC Standards for Records and Record Keeping. Monitor other staffs record keeping and ensure it is done in accordance with the same.
6 Identify service development priorities and implement them, communicating with the matron about all issues.
7 Identify training needs for all staff associated with practice changes/developments and ensure this is carried out.
8 In liaison with the Matron and other professionals identify and lead the development and implementation of relevant clinical guidelines and policies.
9 Ensure adherence to the Trust Medicines Policy and the NMC Standards for The Administration of Medicines.
10 Initiate and lead audits of practice and clinical effectiveness in accordance with Divisional and regional audit plan.
11 Initiate and lead specific areas of practice developments in accordance with local and national policies and strategies to improve patient care.
12 Lead best practice within the clinical area and implement changes in guidelines.
13 Further develop skills continually which are required to practice at an advanced theoretical and practical level.
14 Assess and actively manage within the clinical area in accordance with health and safety legislation and risk management policy.
15 Facilitate and provide access to professional and clinical information for staff in the clinical area.
16 Formulate and ensure the availability of appropriate patient information, including facilitating /ensuring collection of data and act upon audit results.
17 Work within Data Protection and Caldecott requirements for information sharing.
18 Ensure infection control procedures and policy are adhered to and monitored and other staff are aware of their responsibilities in ensuring compliance. Liaise with infection control team as necessary.
ADMINISTRATIVE / MANAGERIAL:
To be responsible for identifying future department needs of Projections and Service development requirements. To be responsible for the day to day organisation of the allocated Stamford Unit Floor. To be responsible for all three floors including staffing on a rota basis with other senior staff. Carrying 24 hour responsibility for the clinical area take responsibility for the planning of the ward rota to meet patient needs, making best use of resources, maintaining appropriate skill mix and reporting any concerns to the Matron. Ensure staff are aware of decisions taken about requirements of the department with regard to off duty, rotas and annual leave. Include and monitor junior sisters in decisions taken, ensuring service needs are met. Competent at use of and understanding of the e-rostering system. Implement and evaluate clinical governance systems ensuring that care is delivered within an appropriate model. Promote clinical excellence and clinical governance using recent and relevant evidence based research. Deputise for the Matron as required. Disseminate implement and ensure adherence to Trust policies by all members of the team. Contribute to the achievement of Trust targets, whilst identifying service development priorities. Contribute to action plans related to external reports. Manage the department pay/non pay budget, in liaison with the Supplies Department and the Matron, working within wider financial and resource/constraints. Ensure staff are also aware of their responsibilities with reference to pay budget and efficient supplies budgeting. Participate in the identification of cost improvements initiatives, including TEP options. When required, having own patient workload, while concurrently troubleshooting, manage and act in an advisory capacity for the multi-disciplinary team. Investigate complaints/accident reports and incidents within the department in line with trust policies. Liaise with matron/PALS re recommendations/actions.
