Are you a qualified Occupational Therapist looking for your next challenge?

We're looking for a Occupational therapist to work as part of the multi-professional Locality Health Team delivering programmes of rehabilitation within the community in close collaboration with other relevant services e.g. other community and inpatient teams, social care, primary and secondary care.

Manage a caseload with supervision as required/be responsible for a defined area of service, undertaking comprehensive assessment, planning and implementation of comprehensive programmes of care for patients who require intensive support / rehabilitation guided by more senior clinicians where required.

Participate in and maintain own clinical professional development (CPD) by keeping abreast of new developments and incorporating into practice under guidance from supervisor and providing supervision and assessment to rehabilitation assistants and students on placement.

Work collaboratively with partners in health and social care, to establish a whole systems approach to managing patients with complex care needs

Main duties of the job

  • Act as a key resource for the Occupational Therapy needs of the wider MDT caseload.
  • To guide the team on the appropriate use of therapeutic risk-taking during MDT discussions, using specialist OT knowledge.
  • To meet the needs of service users in an effective, efficient and recovery focused approach.
  • Provide occupational focused assessment, formulation, care plan, and intervention to patients with complex functional needs in their own homes and communities.
  • Evaluate Occupational Therapy interventions and consider ongoing needs of the patient and identity opportunities for longer term support when required.
  • Chair case conferences and other clinical meetings as indicated.
  • To develop, facilitate and evaluate a group work programme.


About us

We have been working with other local NHS Trusts to create a new organisation, bringing together all community, mental health and learning disability services from across Hampshire and Isle of Wight. Subject to approvals, the aim is for the new Trust to be formed by the end of 2024.

Project Fusion is the name for the programme to create the new combined NHS Foundation Trust. Hampshire and Isle of Wight Healthcare NHS Foundation Trust will be bringing the services into a single organisation will result in more consistent care, more equitable access to services irrespective of postcode, and a more sustainable workforce and services. The new organisation will operate locally to ensure services can best meet the needs of different communities.

The new Trust will be comprised of all the services currently provided by Solent NHS Trust and Southern Health NHS Foundation Trust, the community, mental health and learning disability services provided by Isle of Wight NHS Trust and child and adolescent mental health services delivered in parts of Hampshire by Sussex Partnership NHS Foundation Trust.

Colleagues will transfer to the new organisation under Transfer of Undertakings (Protection of Employment) -- known as TUPE on the day the trust is formed. TUPE is a legal process to ensure you transfer out of one organisation and into another, maintaining the terms and conditions of your employment.

  • Post graduate experience of working with vulnerable adults with complex presentation which includes older people
  • Experience of supporting and supervising junior staff
  • Experience of working as part of a multidisciplinary team
