Planned community services support patients in their home and play a vital role in helping patients to achieve their goals, long past their hospital discharge and manage their long-term conditions in the community independently. You'll help to identify the support patients need and help them to achieve their goals and rediscover their passions. It requires you to think creatively, and tailor interventions for each individual patient. For some, it could be getting up the stairs unaided, going on a bike ride, or being able to cook their favourite dish! Every patient is different, and no two days are the same.
We are a dynamic service providing patient-centred assessment, treatment and rehabilitation to residents of Coventry aged 18 and over. Clinical caseload is varied and challenging including neurological, musculoskeletal, falls, multi-pathology, complex physical needs, including long term conditions and chronic pain. Every patient is unique, and no two days are the same!
Main duties of the job
You will be able to illustrate and reflect upon complex patient management and demonstrate sound clinical reasoning. You will be able to prioritise your workload and time effectively to support appropriate caseload management and delegation.
There will also be opportunities for participating in various service development projects and attending regular in-service training.
At present the Planned Community Therapy service operates from Monday to Friday core hours, however the job description reflects a seven day service in case of changes to service needs.

Physical Effort There is a requirement for o Sitting, standing, and walking during every working day.o Repetitive movements on a regular basis.o Pushing/pulling of wheelchairs or transfer equipment as required.o Moving and handling of equipment daily.o Driving between patient visits
Mental Effort There is a requirement to carry out assessments, perform documentation and provide reports on a daily basis. It is expected that the post holder may be interrupted by carers/family/patient whilst carrying out assessment. It is expected that whilst in the office there may be interruptions from colleagues. There will be a requirement to be able to o Check documents.o Analysing statistics or carrying out calculations.o Carry out assessments and complete documentation.o Maintain contemporaneous patient recordso Preparing reports/letters.o Carrying out non-clinical fault finding.
