Are you a Registered Nurse or an AHP with an interest in digital care technology and would like to utilise your clinical skills to support a teaching/training role in care homes as an autonomous practitioner?
The Care Home Support Team are enthusiastic committed healthcare professionals that have built excellence working relationships with the care homes in the Borough of Richmond. The team are proud of the service that they have developed and its expansion in improving proactive care for residents, hospital avoidance and for the support and training provided to care home staff.
The Care Home Support Team in Richmond have an exciting new opportunity for you to join our dynamic team as a clinical and digital practice educator. You will be providing training to support the embedding of remote monitoring and other clinical and digital care technology in care homes. Using your knowledge and skills of good clinical practice to train care home staff in clinical education and how to recognise early signs of deterioration using the NEWS2/RESTORE2 framework and how new technology can support this.
We have multiple digital integration projects currently in implementation with care homes. our aim is to reduce avoidable LAS call outs/hospital admissions working in partnership with the SWL ICB and PCN's. You will identify barriers, seek to understand the challenges care homes present and identify solutions to good digital integration and clinical education.

Main duties of the job
- Support care homes with changing clinical practice to integrate core Bluetooth enabled remote monitoring
- To support the increase of care homes signing up to the remote monitoring project
- Provide support to care home staff in achieving appropriate clinical competencies
- To embed remote monitoring, assessment escalation and communication tools into practice
- To have an excellent understanding of the Restore 2 process to support teaching
- Increase digital inclusion and mitigate against digital exclusion and inequality by addressing barriers in the care homes such as access to digital equipment, skills and confidence using digital equipment and a lack of motivation to change current processes and day to day clinical activities.
