An exciting opportunity has arisen within the East Dorset ICRT Team offering a position within the existing team. This vacancy post will be required to deliver an Admission Avoidance, Discharge to Assess and Rehab Therapy Service to patients within the East Dorset Locality Area.
You should be a committed, dynamic, motivated professional having enhanced assessing skills, the ability to prioritise your work and be a good communicator. The post holder will be required to take some management responsibilities within the team.
Recent experience of working in a similar environment would be an advantage. Awareness of the effects of ill health and the ageing process and a basic level of IT literacy are an essential requirement.
The post is based within the East Dorset Locality the post holder will need to have a current UK Driving Licence and access to a car, as they will be required to monitor patients in their own homes and other community settings.
Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.
Main duties of the job
The post holder will be expected to have sound knowledge in order to recognise the deteriorating patient and be able to escalate appropriately to senior colleagues or emergency services. This will include awareness of basic observations, pressure care management, medication, continence.
The post holder will need to demonstrate multi-disciplinary skills in order to undertake functional and holistic assessments that are evidence based and formulate patient centred goals to promote independence. To formulate treatment plans and delegate these to junior assistant staff where appropriate.
To manage own caseload ensuring prioritisation and timely input to patients that have been referred to the service.
To support the MDT to clinically triage incoming MDT referrals and allocate to a waiting list appropriately. A sound knowledge of other Health and Social care roles is required.
To demonstrate sound understanding of clinical governance by ensuring practice is evidence based and up to date. To actively seek learning opportunities through training, supervision and shadowing to achieve this.
To support Nursing students under the supervision of a senior Nurse.
Be responsible and accountable for all aspects of the practitioner's professional activities in accordance with the Nursing and Midwifery Council (NMC)
