The Medicines Management Pharmacy Team at University Hospitals of Derby and Burton have an exciting opportunity to join the pharmacy team at the Royal Derby Hospital as a ward based Advanced Medicines Management Technician working in Cardiology speciality.

This band 6 role requires the Level 4 Clinical Diploma being held. However, we welcome applications from candidates willing to work towards this qualification and we would support them in a band 5 Medicines Management role until successful completion of the diploma, which would then lead to automatic progression to the band 6 Advanced Medicines Management role. We are looking for a Pharmacy Technician to join our team who has substantive post qualification experience and has been working in a clinical role.

Main duties of the job

The Advanced Medicine Management role will provide an enhanced clinical ward based Medicine Management and Optimisation Service, including medicine reconciliation, patient counselling, medication reviews ,and timely supply of discharge prescriptions.
  • Ensuring the quality, accuracy and safety of prescribing in clinical area.
  • Supporting patient safety by actively contributing to clinical decisions alongside pharmacists and other prescribers
  • Obtaining detailed and accurate medication histories from multiple sources and reconciling with the inpatient prescription and communicating any discrepancies.
  • To fully support and deliver referrals via the DMS, to aid the transfer of patients between care settings.
  • Reviewing inpatient and discharge prescriptions to ensure appropriate prescribing and availability of medicines.
  • Assessing Patients own Drugs for suitability for use in the Trust.
  • Promoting use of Trust and national clinical guidelines and optimising patient medication.
  • Help support quality, safety and timeliness of discharge and to facilitate effective communication of medication around admissions and discharges.
  • Using blood results and other results available, review prescribed medications, highlighting areas requiring change.
  • Provides a safe and efficient supply of pharmaceutical products to patients. Including final checking (ACT) of drug products and compliance aids.
