To work as a member of the Community Health Care Team

To assess, plan, implement and evaluate care under the direction of the team leader.

To ensure the provision of effective, efficient, appropriate, high quality patient care over 24 hours, seven days a week.

To deputise for the District Nursing Team Manager/Deputy in their absence.

To act in accordance with the NMC Professional Code of Conduct and other policies.

Main duties of the job

Two registered nurses permanent posts have become available within the North West District Nursing Team for staff nurses who wishes to develop their nursing career within the exciting speciality of community nursing.

Applications are invited from motivated and enthusiastic individuals with a flexible approach who are committed to providing compassionate nursing care of the highest standards to people within their own home and local community.

Individualised education packages will be developed if you are new to the community service. Opportunities for career progression available for successful candidate.

The ability to speak Welsh is desirable for this post Welsh and/or English speakers are equally welcome to apply.
