This role will lead the development of integrated ways of working across all providers supporting the health and care for the population of three localities - with a specific focus to develop new models of care. The post holder will be responsible for ensuring the implementation and transformation of service delivery across those Localities.
The post is anticipated to evolve to support the development of the Community Care Collaborative and will enable the development and mobilisation of an integrated Locality Operating Model.
Main duties of the job
To provide operational transformational leadership across three Localities and develop joint working between primary care and Birmingham Community Healthcare, Local Authority, Acute, Mental Health and Local Authority services by establishing effective working relationships.
To provide strong visible, inclusive and compassionate leadership across the Localities.
To deputise for the Associate Director of Transformation as requested on specific transformational workstreams.
To oversee the Locality Managers portfolio, developing effective delivery strategies are in place for all Localities within the postholders portfolio.
To ensure processes and systems are in place to monitor the performance and activity of all Localities within the postholders portfolio.
In collaboration with the Locality Managers promote a culture of delivery and collective working whilst ensuring robust communication systems exist for the dissemination of information to teams/stakeholders across Localities.

To be responsible for the Locality Managers delivery in the development of locality partnership, in liaison with multiple organisations, to implement the delivery of Community Care Collaborative workstreams
o Integrated Teams in Localities and Neighbourhoods

To act as the point of escalation for Locality Integration Managers and provide senior oversight to develop the Locality Partnership across the three Localities within the postholders portfolio, ensuring they act as the vehicle for the delivery of integrated community services in the respective locality for the benefit of the population.
To provide senior support to the Operational colleagues when developing revised demand and capacity profiling across multiple Services and Localities within the Community Care Collaborative programme, ensuring workforce plans are revised for the Localities within the postholders portfolio.
To work in conjunction with senior colleagues within the Community Care Collaborative and oversee the development of reporting functions, at a Locality Level, which will then be reported to the various Locality Delivery Partnerships.
To co-ordinate the scoping and delivery of the Community Care Collaborative workstreams across providers with clear articulation of the added benefit for partners and citizens. This will include the development of an outcomes framework to be applied at locality level for the Localities within the postholder portfolios.
To provide Senior oversight of the development of an annual plan for all Localities within the postholders portfolio that align with the wider Community Care Collaborative and ICS priorities.
To work in collaboration with fellow Senior Locality Integration Manager who is also responsible for several Localities to ensure consistency where practically possible in systems and processes supporting the Community Care Collaborative Programmes/Teams.
Establish and maintain the work programme, planning, reporting and supporting governance framework for the three localities within the postholders portfolio.
To build relationships with senior colleagues in Divisions, to resolve and escalate any issues identified externally impacting upon effective locality joint working/ delivery of patient services and progress service developments and projects as appropriate relating to locality service improvement.
When working on improvement project with the locality partners advocate and embed a continuous improvement culture.
Drafting reports summarising status on issues, appraising outcomes, and providing progress reports for the Associate Director of Transformation where appropriate.
Collate as required, complex qualitative and quantitative information and lead appropriate in-depth analysis to develop robust business cases and progress of Community Care Collaborative workstreams across multiple Localities.
Analyse, interpret and present highly complex data related to health and care outcomes for multiple locality populations to highlight issues, risks and support decision making to present to Locality Delivery Partnership associated Committees/Board
Understanding of Population Health Management and implications for place-based working. This will involve planning and organising a broad range of complex activities and formulating and adjusting plans or strategies including long-term strategic plans, service improvement plans, involving uncertainty, which may impact across the whole locality / system.
