The advertised post is for a band 6 specialist nurse within the Crisis Intervention Team for Older People (CITOP). The CITOP team currently operate between 8am and Midnight.
The post requires providing crisis and home treatment support to adults predominately aged over 65 in the Hull and East Riding area. The team supports patients with both functional and organic illness.
The role will involve triage and assessment of those new into services as well as increased care provision to those already open to other teams.
Main duties of the job
The applicant will be expected to have specialist nursing knowledge of working with older people with organic and functional mental health issues.
You will have the opportunity to develop leadership skills , providing supervision to healthcare staff . You will lead and participate in the development of staff through appraisal and clinical supervision.
You will provide nursing interventions across Hull and East Riding, in order to support the wider community services as required by patient need and demand. You will provide care as an alternative to hospital admission or to facilitate a timely discharge from inpatient settings. You will be required to triage and assess patients who require a urgent or crisis response. These assessments will be completed in a range of community setting. You will be required to collect/collate information, and attend formulations for identified patients. You will also be required to complete 'gate keeping assessments' and liaise with the bed management team around admissions. You will be required to work in collaboration with other teams and agencies to ensure the best care for the teams patients.
The applicant will be expected to have knowledge in mental health conditions, including Dementia and experience in this area would be an advantage, additional training and support will be given.
