We are recruiting for a Community Nurse who wishes to join out team. You should share our passion and drive to ensure that our patients and staff receive quality and compassionate care and work with us to ensure the Trust continues its journey towards great people, great place.

We are looking for a Registered Nurse to join our dynamic Integrated Community Nursing Service. Community nursing is changing rapidly we provide all aspects of nursing care at home for many patients who would have traditionally remained in hospital.

The three locality teams work closely with our colleagues from Earl Mountbatten Hospice, Community Rapid Response, Occupational Therapists, Physiotherapists, GPs, Practice Nurses, Specialist Nurses, mainland hospitals and outside agencies, such as Age UK and the Local Authority.

We would expect you to have good interpersonal skills and be able to demonstrate that you are an evidence-based practitioner.

We work across three locality teams and the position will be for one of the three localities depending on the service need. Any preferences should be made know an interview however this cannot be guaranteed.

It is essential that you have a valid driving licence to drive in the United Kingdom as the role is community based.

Clinical skills in all types of urinary catheterisation, syringe driver management, wound management and diabetes management are desirable, but not essential.

Support and training will be available to ensure safe a safe transition to working in the community setting.

A preceptorship programme is available.

The final combination of Southern Health and Solent Trusts is anticipated to occur in July 2024, when the new organisation will be formed.

Qualifications

Essential

  • Registered nurse with current NMC registration


Experience

Essential

  • Awareness of current issues in community nursing
  • Clinically up-to-date
  • Knowledge of the principles of manual handling and blood borne disease


Additional Criteria

  • Physically capable of performing the role and responsibilities expected of the post holder, e.g. car driver, lone working
  • Car driver with clean licence
  • Awareness of personal safety
