Community nursing teams deliver holistic, complex care to housebound patients within their own home environments.
  • Do you have a passion for community nursing?

Main duties of the job

Working in collaboration with colleagues and the wider leadership team you will:

-You will be responsible for allocating patient visits within the skill set for the community nursing team. You will work alongside the Lead Practitioner to manage the caseload, including reviewing demand and capacity.

-You will be competent in a wide variety of clinical skills, and be able to share your knowledge and expertise with others, signposting to more specialist services as appropriate.

-You will an effective leader, demonstrating care, compassion and empathy and role modelling the Trust values. You will be able to support staff within the team to develop their own skills and will offer a high standard of supervision.

-You will be able to engage complex patients, agree mutually beneficial treatment plans and to plan, implement and evaluate care pathways.

-You will support with quality assurance, including audit, incident reporting and gaining patient feedback. You will work with the leadership team to evaluate this in line with objectives.
