District Nurse Team Lead - Kington

We are the Kington & Weobley District Nursing Team, a small, dedicated team providing excellent planned and unplanned nursing care to patients in their own homes. We aim to deliver high quality care to improve health & well-being and to support our patients to remain living at home. We pride ourselves in being hard-working, supportive, friendly and flexible. We are part of the North & West Primary Care Network and work closely with partners across the network.

We are based in the market town of Kington nestled within the beautiful County of Herefordshire - covering both Kington and Weobley surgeries in the north-west area of the county. This rural area has many areas of outstanding beauty and is predominantly agricultural with rolling country side for miles.

We are looking for a team leader to join our team who shares our passion for caring for people in their own homes, strives towards excellence in community nursing and with a keen interest in integrated working and quality improvement. We're keen to recruit a colleague with the right values and behaviours who can help us deliver our Trust priorities.

Main duties of the job

You will be responsible for the overall management of the team caseload and the rota. Ensuring the right skill mix and workforce development are key elements of the role.

You will lead on recruitment, training and reporting supported by the Quality Matron and Locality Manager. You will undertake some clinical work in a wide range of nursing interventions.

This roles requires you to have excellent organisational skills, an ability to build relationships with internal and external colleagues and an appetite for innovation. You should be able to evidence the ability to lead and motivate others and be able to demonstrate excellent communication and interpersonal skills. Good IT skills are a must!

Whilst we value the Specialist Practitioner Qualification (SPQ), we will consider applications from those without it, but may have something at an equivalent level/relevant experience. It is the leadership ability that we need to support the team and our patients.

It includes working weekends and bank holidays as part of a rota and late shifts until 8pm. You must be able to drive and have access to a car. There are opportunities for lease cars.

If you have the right values for the WVT you will be supported by a programme of continuous training and development. The position offers opportunities to get involved in Quality Improvement & audits.

Person Specification

Education & Qualifications

Essential

  • RN qualification registration part 1 of the NMC register
  • DN specialist Practitioner Degree recorded with the NMC
  • Non- medical prescriber recorded with the NMC


Desirable

  • Masters level Health Assessment Module


Skills, Knowledge & Abilities

Essential

  • Teaching and developing of educational packages for nurses other HCPs and patients
  • Collaborative organisational skills
  • Effective MDT working
  • IT skills
  • Significant knowledge of the profession and its development in a national context
