The flagship dementia wards including physical health and frailty based on Kingsway site Derby are offering a role for a qualified Advanced Clinical Practitioner (ACP).
The successful ACP will work closely with the multidisciplinary team, sharing leadership with other senior clinicians and managers using specialist knowledge and skills particularly within physical health to provide autonomous care to patients within the older adult inpatient services.
You will practice independently undertake physical and mental health examinations as indicated by the individual's presenting condition autonomously analyse complex clinical signs and investigation results to consider differential diagnoses, formulate decisions, and diagnose conditions, initiating treatment as required - deciding when necessary to refer to senior medical colleagues.
The post holder will be a registered nurse or AHP and hold an MSc in Advanced Clinical Practice or equivalent at MSc level together with a portfolio that meets the required standard of the Derbyshire Competency Framework with experience within physical healthcare.
You will demonstrate an in depth knowledge and act as role model to strengthen the physical healthcare provided on the ward alongside the support provided by a Consultant Geriatrician. You will provide clinical leadership to the ward team and be a valued member of the medical team.
Main duties of the job
All Advanced Clinical Practitioners (ACPs) should be competent to practice all core role competencies and, as such, be capable of working across all clinical areas to the standards established within the Derbyshire Advanced Practice Network. However, there will be variations in the level of skill above this standard, depending on the specialist area of practice. ACPs are expected to support the establishment and delivery of high-quality, evidence-based care.
- Independently conduct advanced practitioner-level holistic assessments, planning, implementation, and evaluation of complex mental and physical healthcare needs for working-age adults and older adults.

DHCFT have a growing number of ACP's and are part of the wider Derbyshire Advanced Practice system.

- They will be an independent Non-Medical Prescriber registered with the NMC or HCPC.
