An exciting opportunity has arisen to come and join our supportive, dynamic social care team within the South West (Cannock & Seisdon and Stafford) Discharge to assess service. The social work team working in partnership with health colleagues (Therapy and Nursing) supporting individuals who have moved from hospital in to a temporary D2A bed in designated care homes in Cannock / Seisdon.

We will support you through the Social Work Learning Academy and encourage you to develop personally and professionally via monthly supervision and yearly appraisal. Via the Social Work Learning Academy (SWLA) you will have access to a range of courses / training to support your development as a social care practitioner. There is also the opportunity to progress and apply for the social work apprentice degree if you wished to follow that route.

Main duties of the job

You will complete Care Act assessments / support planning, to support individuals when their long term care needs have been identified or support individuals with information, advice and guidance.

Our service currently works Monday to Friday, but we also operate flexible working arrangements such as condensed hours

Social Work Learning Academy (SWLA)

On appointment, the post holder will be auto-enrolled in the SWLA and aligned to the appropriate British Association of Social Work Professional Capabilities Framework. The relevant SWLA 'stage' (i.e. stage 1 (ASYE), 2 (Social Worker) or 3 (Advanced Social Worker) will also be agreed taking all prior Continuous Professional Development (CPD) and relevant experience into consideration. The post holder will be required to take responsibility for their own Continuous Professional Development.

Person Specification

QUALIFICATIONS & TRAINING

Essential

  • Level 3 Diploma in Adult Care or Lead Adult Care / Social Care Worker
  • Level 4 Diploma in Adult Care or Lead Practitioners in Adult Care Level or willing to complete / achieve this if a Level 3 qualification is already held


Desirable

  • Work based supervisor training (Level 3)


Experience

Essential

  • The ability to demonstrate a commitment to working in partnership with adults, carers, professionals and voluntary agencies, to provide a comprehensive service which meets the needs of the adults in line with current legislation and directorate, local authority & Trust policies
  • Experience of working in a team, providing formal support and supervision to junior colleagues, incorporating good interpersonal relationships and the ability to actively contribute to the implementation, development and review of an individual's care and support plan
  • Experience of working within the specialist area
  • Experience of working in an NHS/Local Authority setting where MDT and specialised practice associated to social work and social care is required
  • Experience in dealing with complex casework involving family members, carers, communities and other organisations


Desirable

  • Experience of providing informal and formal staff supervision
