An exciting opportunity has arisen for a highly motivated, innovative and experienced Nurse to join the Wandsworth Learning Disability Healthcare Team.
The successful candidate will have a substantial experience in working with people with intellectual disabilities with complex health needs including epilepsy, dementia, challenging behaviour and mental health.
The role is funded as a 12-month pilot which will focus on implementing the NHS England Enhanced Health in Care Homes (EHCH) framework in Wandsworth Care Homes. The post holder will work collaboratively with care homes to scope, identify and develop pathways which improve the health outcomes and address health inequalities for residents. With support of the Clinical Manager and Lead Nurse in Intellectual Disabilities and Autism, the post holder will audit, review and report on outcomes against set key performance indicators for the pilot.
Main duties of the job
The post holder will work as an autonomous practitioner, using specialist clinical skills, liaising effectively with all stakeholders and delivering training to care home staff and the network.
The post holder will be an integral member of a diverse multidisciplinary service which provides expert, caring and holistic support.
You will have access to professional supervision from the Learning Disability and will be actively encouraged to participate in continuing professional development activities commensurate with your post. There is excellent staff support, regular appraisal and CPD.
To work collaboratively with care homes in Wandsworth to meet the health needs of people with learning disabilities,reducing or preventing the need for additional health services.
To focus on implementing the NHS England Enhanced Health in Care Home (EHCH) framework and utilise learning from the locally commissioned report for Wandsworth on Covid Recovery (2022).
To focus on identifying and addressing health inequalities.
To work as an autonomous practitioner, using specialist clinical skills, liaising effectively with all stakeholders and delivering training to care home staff and the network.

Job responsibilities
To be an effective role model for clinical excellence. Demonstrate visible leadership and support care homes to deliver care in line with best practice.
To assess, advise and contribute to meeting the health needs of people with a learning disability in a care home setting.
Introduce a Training Analysis of care home staff to identify areas of further development and educative support
Support training and education in line with EHCH framework including but not limited to constipation, nutrition and hydration, mouthcare matters, continence and the benefits of physical and meaningful activity.
Facilitate learning opportunities for care home staff, both formal and informal
Promote and support up-take of Annual Health Checks with GP Practices/PCN networks
Engagement in physical and meaningful activity
Review quality of Health Action Plans/Health Care Passports advise/support/facilitate identified health needs and signpost to appropriate services. Including specialist services for dementia, mental health and behavioral support services.
Liaise with GP services to formulate and implement Universal Care Planning in line with current guidance. Advocate for residents to ensure they are the center of all decision-making.
Support staff with MCA/BI decision making with GP services/involved health professionals and families.
Support care homes to develop systems to promote the active engagement of families and informal carers in care and service developments.
Support care homes with hospital admission and complex discharge planning. To reduce failed discharges and recurrent hospital admissions promote Red Bag initiative.
Support care homes to develop and embed processes which support the delivery of multidisciplinary team meeting as outlined in the EHCH framework
Provide regular reports for LeDeR via the LAC and steering group.
To contribute to multi-disciplinary planning and meetings coordinating appropriate information and documentation
To review and promote engagement of care homes in Structured Medication Reviews

Training & Qualifications
Essential
- RNLD (current NMC registration)
- Evidence of continuing professional and personal development
- ENB 998 or equivalent
Desirable
- Education to Masters level or equivalent
Experience
Essential
- A minimum of 3 years experience of working as a learning disability nurse
- Experience of working in a community setting
- Experience of assessing the needs of people with learning disabilities and developing a robust care plans
- Experience of teaching and training of staff and/or carers
- Experience of contributing to policy and procedure development and implementation
Desirable
- Experience of developing and delivering training packages with other professionals
Knowledge & Skills
Essential
- Knowledge of the predominant health needs of people with learning disabilities
