Main duties of the job
- Patient Engagement: Develop trusting relationships with patients by giving them time and focusing on 'what matters to me'. Facilitate their journey on the clinical pathway by providing personalised support.
- Pathway Coordination: Monitor and track patients' journeys, working closely with the multidisciplinary team (MDT) to ensure their individual needs are supported and met throughout.
- Collaboration: Build strong partnerships with third sector organisations, health and social care providers.
- Outreach and Insights: Conduct direct outreach with patients, community groups, system partners, and clinicians to provide holistic insights and enable targeted planning and improvement of inequality outcomes.
- Caseload Management: Manage and prioritise a caseload based on the needs, priorities, and support required by individuals.
- Leadership and Improvement: Role model compassionate and inclusive leadership, promoting a culture of collective leadership within the trust.
- Service Knowledge: Maintain an in-depth understanding of the range of support services available within health and social care, guiding patients to the services most likely to meet their needs.
- Referral Pathways: Identify new referral pathways and strengthen existing ones, forging strong links with local VCS organisations, community, and neighbourhood-level groups.
- Support and Signposting: Help patients address issues impacting their health and wellbeing by signposting them to appropriate support.

- To develop rapport with patients to identify need and facilitate support.
- To support patients to find and access the right information and appropriate signposting to services.
- To identify and manage any risks relating to patient contact, particularly when engaging with patients alone.
- Resolution of patient presenting issues and pro-active management and escalation of any risks.
- Provide personalised support and advice meet people on a one-to-one by phone, video conference, or face-to-face. Give people time to tell their stories and focus on what matters to the person.
- Build trust and respect with the person, providing non-judgemental and non-discriminatory support, respecting diversity and lifestyle choices.

- Previous experience in a health coach role or similar public health role, community development or social prescribing up to degree level or equivalent
Desirable
- Health/social care/community development related degree or equivalent
- Personalised Care Institute accredited health coaching qualification/training in motivational interviewing/active conversations
- RSPH Level 2 Award in Supporting Behaviour Change (Health and Wellbeing) or RSPH
- Level 3 Diploma in Health and Wellbeing Improvement - Supporting behaviour change in professional practice

- Be able to meet the travel requirements of the role which involve travel between trust sites and partner, and community locations.
- You may also need to travel between Trust premises as required for the performance of your duties. You may also be expected to work at any of the trust sites. The Trust would consider making reasonable adjustments to the role, if necessary, to enable a disabled person to undertake the role.
