GLOUCESTER and FOREST OF DEAN ASSERTIVE OUTREACH TEAM
Social Worker Band 5/6 progression post
This is an exciting opportunity for a suitably qualified Social Worker to gain experience working with a busy but dynamic Assertive Outreach team.
The Gloucester and Forest of Dean Assertive Outreach Team provides comprehensive mental health and social care to working age adults in the Gloucester locality and the Forest of Dean.
We are seeking a motivated and innovative individual to join this enthusiastic, progressive and dynamic multi-disciplinary team. We can offer an opportunity to develop your skills and experience and to contribute to the development of a high quality service.
The team provides a planned needs lead 8am-8pm seven days a week service (reduced hours at weekend). Our core working hours at Monday - Friday 9am - 5pm and 9am - 1pm weekends and Bank holidays. The hours are 37.5 per week with some evenings and weekend shifts (on a rotational shift system). 'Flexible' working arrangements will be considered. Any request will be set against genuine operational requirements and limitations and it is not obligatory for a manager to accede to the request.

Main duties of the job
- To work as a Care Co-ordinator or Lead Professional (where applicable) with individuals who have serious and enduring mental health difficulties.
- To develop and provide evidence based programmes of care for service users of working age with mental health problems who are receiving treatment from the Recovery Team, with a focus on social care
- To ensure a clear focus on recovery and social inclusion.
- To support and supervise both registered and non-registered staff in delivering care.
- The role will entail, assessment , support planning and review of social care packages in robust and timely manner, in accordance with The Care Act 2014 requirements
