The Memory Assessment Services are a community team who are looking to recruit an enthusiastic Occupational Therapist with a passion for working with people living with Dementia. The Team has gone through a recent Transformation Process and is currently working with Community Partners in a Whole Person Approach.
The Role of an Occupational Therapist was recognised within the transformation process as a vital part of the team, in order to deliver excellent care to the people we support, pre and post assessment.
The successful candidate will join a team that is committed to ensuring that the person under the care of the Memory Assessment Service will receive excellent and compassionate care. Clinical activities take place primarily in people's homes and can include other community settings such as care homes and other supported living residences.
Education and development are seen as an essential part of each role within the team and as such, The Memory Assessment Services are committed to supporting the development of everyone in the team. Opportunities for development will be discussed with the Team Lead.

Main duties of the job
As part of the Memory Assessment Service, you will be working within a multi-disciplinary team that is consultant led and practitioner delivered. Your expertise as an Occupational Therapist will be valued and integral to the development of the services to support people living with Dementia including Young Onset Dementia (people under the age of 65).
As a member of our team, you will be based within the Memory Assessment Service and will work alongside another Specialist Occupational Therapist, Nurse Consultant, Advanced Clinical Practitioner, Nurse Assessors, Consultant Psychiatrists, Associate Practitioners with an efficient administrative support team to provide a multi-disciplinary service for people with dementia, their families and/or their Carers. In addition to the Memory Assessment Services, you will work in partnership with The Admiral Nurses and other Community Services. The role will include working autonomously as well as collaboratively within the wider team.
Clinical activities take place primarily in peoples' home and can include other community settings such as care homes and other supported living residences.
