Band 6 OT ROLE DFG/HOUSING up to 32 hours
We have an exciting opportunity for a Band 6 Occupational Therapist. We are seeking an experienced OT to focus on Housing adaptations and the progression of Disabled Facilities Grants.
Working closely with PCC Community Connections (Housing), contractors and Housing Associations you will use your OT skills and knowledge to ensure adaptations are completed in a person centred way, which meets the persons longer term needs and to review once completed to ensure assessed need has been met.
Ideally, you will have relevant, recent experience in relation to Major adaptations and be able to work creatively with Housing colleagues to find solutions where the environment is impacting on a persons Independence, function and wellbeing. This may also involve assessment and matching of void properties as a more suitable outcome.
Developing and sharing your expertise, you will work closely with the wider, Integrated Therapy teams to share your knowledge and build confidence and skills across the Organisation in relation to Major Adaptations, legislation, eligibility and relevant processes.
Main office is Crownhill Court, but post holders can work from any Livewell base and home-working.

Job responsibilities
- To work flexibly within MDTs enabling provision of a quality Occupational Therapy (OT) service within various settings and services including: community inpatient, outpatient, group work, intermediate care, rehabilitation and other health and social care services provided within the Community Therapy service model.
- To be responsible for the advanced functional assessment and treatment of people with complex and/or chronic presentation, using evidence based / patient centred principles to assess, plan implement and evaluate interventions
- To participate in the planning, development and evaluation of OT services within a designated area/team, holding responsibility for defined projects
- To regularly supervise OT students on practice placement
- To deputise in the absence of the Team Lead and carry out operational / leadership duties as appropriate
- To provide leadership for Band 5 and support staff through supervision and appraisal and facilitate them to achieve their competencies and personal development plans.
- The post holder works flexibly to cover 7 day working as required.
- To work flexibly as the service requires to cover across the community areas provided by Livewell Southwest in times of sickness, annual leave and training.
