Are you interested in and enthusiastic about mental health pharmacy or wanting to specialise in this field?

We're looking for an experienced Advanced Clinical Mental Health Pharmacist to play a key role across our Primary Care Network (PCN) based throughout Great Yarmouth, Gorleston, Lowestoft & South Waveney.

Our Primary Care Network (PCN) Service for Mental Health has been developed to provide access to mental health services in Primary Care as part of an integrated health and care system, which brings care closer to home for our population.

We're working collaboratively with the PCN to support the redesign and transformation of primary and community mental health care to develop and deliver a new service paradigm in line with the NHSE community mental health transformation agenda.

As PCN Advanced Clinical Mental Health Pharmacist, you'll play a vital role in supporting the integration work to implement the transformation by developing and providing medicines optimisation services to the people in the Great Yarmouth and Waveney area.

The ideal candidate will need to have a strong background in mental health as well as being confident working independently. You'll be working across all 4 PCN sites, so will need to have a flexible and proactive approach to your work.

Main duties of the job

The PCN Advanced Clinical Mental Health Pharmacist will work as part of the Primary Care Mental Health Team to provide clinical services directly to patients and service users as well as supporting a system approach to mental health medicines optimisation and management.

Main duties include:
  • Support the upskilling of PCN and practice pharmacy team to increase their confidence and skills managing medicines in this patient group.
  • Direct pharmacy teams to complete mental health safety and prescribing audits
  • Improve prescribing in line with NICE guidelines and NHS England STOMP and BPSD initiatives
  • SMI reviews, physical health monitoring and health promotion
  • Reduce inappropriate polypharmacy, side effect monitoring and reduce inappropriate antipsychotic prescribing in eg. dementia
  • Be a new referral pathway for GP's. MHP's and pharmacists within the community setting.
  • Provide specialist advice on medicines for mental health conditions

Qualifications
Essential
  • Registration with GPhC
  • Postgraduate certificate and/or diploma in Psychiatric Pharmacy or other post-graduate clinical pharmacy qualification including mental health modules

Desirable
  • Independent Prescriber or working towards an Independent Prescribing

Experience

  • Postgraduate employment as a pharmacist
