Portsmouth OPMH (Older Person's Mental Health) team are looking for a motivated, enthusiastic practitioner to join our friendly team, who provide skilled assessments and treatment interventions to older adults.
To be a successful applicant you will need to be a registered nurse, preferably with some experience in care of older persons with mental health problems. You must be genuinely interested in in working with older adults and provide compassionate care to patients and their families with knowledge of organic and functional illness with a good understanding of physical health problems.
You will play an important role in the team as we aspire to provide excellent care to our patients and develop our service in line with national and local guidance. You will be expected to work within the wider MDT implementing and evaluating care, caseload management. You will supervise junior staff and students within the team. This is a really exciting time in joining our team as the Mental community services as we are going through a transformation and you will have an opportunity taking part in transforming our service.
A full driving license is required.

We have been working with other local NHS Trusts to create a new organisation, bringing together all community, mental health and learning disability services from across Hampshire and Isle of Wight. Subject to approvals, the aim is for the new Trust to be formed by July 2024.
Project Fusion is the name for the programme to create the new combined NHS Foundation Trust. Hampshire and Isle of Wight Healthcare NHS Foundation Trust will be bringing the services into a single organisation will result in more consistent care, more equitable access to services irrespective of postcode, and a more sustainable workforce and services. The new organisation will operate locally to ensure services can best meet the needs of different communities.
The new Trust will be comprised of all the services currently provided by Solent NHS Trust and Southern Health NHS Foundation Trust, the community, mental health and learning disability services provided by Isle of Wight NHS Trust and child and adolescent mental health services delivered in parts of Hampshire by Sussex Partnership NHS Foundation Trust.
Colleagues will transfer to the new organisation under Transfer of Undertakings (Protection of Employment) -- known as TUPE on 1st July 2024. TUPE is a legal process to ensure you transfer out of one organisation and into another, maintaining the terms and conditions of your employment.

Main duties of the job
- Assess patients/clients, plan develop/implement/evaluate programmes of care and carry out nursing procedures.
- Provides direct nursing care for patient/clients to achieve best health outcomes.
- Ensures use of nursing knowledge and skills to assess, plan, implement and evaluate patients/clients/family/health needs as well as actively promote health and wellbeing.
- Evaluate clinical outcomes and reports variance to clinical pathway, protocols, or best practice guidelines.
- Contributes to effective discharge planning and/or referral processes to health care providers and support agencies to meet identified health needs.
- Applies diagnostic reasoning and professional judgement to nursing practice issues/decisions.
- Manage care of patients/clients on a day to day basis and when required manage care of all patients/clients in the absence of senior colleagues.
- Fosters inquiry and critical thinking amongst colleagues to advance nursing practice and patient/client care to ensure the provision of positive patient/client outcomes and person-centred care.
