Under the Community Forensic Services the Assertive Transition Service operates as an in reach/community service to secure forensic hospitals supporting Mental Health and LDA patients who are transitioning from secure care to the community providing support for up to 6 months pre discharge and 6 months post discharge.

We work in an alliance with Framework, Rethink and St Andrews across the East Midlands. There are 2 teams the South team are based at St Andrews hospital in Northampton who cover Northampton and Leicester and the North team are based at Westminster House in Nottingham and cover Nottingham, Derby and Lincoln.

You will contribute to the development of bespoke patient plans, work on the daily living skill needs of individuals, interface between patients' clinical teams and community teams and contribute to the production of forensic risk assessments. You will liaise with other agencies and parties involved in the discharge process and support the co-ordination of the elements of a patient's care that will need to come together to support successful transition into the community. The role will ensure that patient support and interventions will continue into the community for up to 6 months under their designated care team.

Main duties of the job

The successful candidate will work as part of a community based multi-agency Assertive Transition Service.

The role requires an experienced nurse with specialist skills to assess patient's needs, analyse and formulate care plans within a multi-disciplinary framework and to support transition to the community, experience of working within forensic services or with individuals diagnosed with a Learning Disability and/or ASD.

You will be required to travel across the 3 counties working into NHS and private providers to co-ordinate the transition into the community linking in with the hospital MDT and identified community team to ensure a safe and successful discharge.

A full UK driving licence and vehicle for business use is required for this post however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.

Job description

Job responsibilities

We are looking for an experienced registered nurse (RLDN) with current registration with the nursing and midwifery Council (NMC).

The Assertive Transition Service will work to expedite the safe, smooth discharge of Forensic Learning Disability and Autism patients from secure hospital into the community, by providing earlier support in a persons discharge pathway and more robust care and support post-discharge. The teams will consist of multi-disciplinary health and third sector agency and peer support staff to form one integrated service

You will work carrying a complex caseload over Notts, Derbys and Lincs and provide in reach work to secure and community settings to support the transition of individuals leaving hospital.

You will have sound knowledge and understanding of working with individuals with a recognised IDD and/or ASD , with a forensic risk. The importance of working with colleagues from multiple agencies to ensure a robust package of care is available.

You will work in partnerships with IMPACT case managers, Transforming Care, Community Forensic Services, Adult Mental Health, Private Sector and Local Authorities.

You will work directly with individuals and their families and carer groups.

You will deliver person specific interventions inclusive of training packages

You will have a sound knowledge of Mental health legislation including MHA, MCA including DoLs.

Person Specification

Experience

Essential

  • Experience of working in or closely with a multi-disciplinary team in either IDD services or Forensic Mental Health Team.
  • Previous experience with adults who have complex needs
  • Show an understanding of Care Programme Approach for clients in the community
  • Able to demonstrate successful multi-disciplinary working
  • Experience of leading clinical assessments and providing high quality feedback.


Desirable

  • Demonstrate working knowledge of Mental Health Act (1983) MCA and DOLS
  • Awareness of different assessment tools available i.e. HoNOS-LD, HCR20v3
  • Experience of working in Forensic environments.
