The post holder is responsible as manager, for the smooth and efficient management and day to day running of the ward and monitors the assessment, development, implementation and evaluation of programmes of care for patients.

To oversee and ensure high quality efficient and effective service delivery on the ward, meeting defined performance indicators including quality standards, quality goals and patient experience.

To manage and monitor activity and performance on the ward and escalating issues appropriately to the Service Lead

The post involves both management and the individual professional development of the Multi-disciplinary team incorporating the Trust's appraisal system.

Main duties of the job

  • To support the Service Lead in achieving a high quality of delivery, monitoring, reporting and development of the service.
  • To be responsible and accountable for the delivery of the clinical care on the ward, taking delegated responsibility for operational management and co-ordination of clinical and administrative staff.
  • To work autonomously and in a self-directed way within the parameters set for the post. They will prioritise workload and make the necessary contributions to ensure delivery of an effective and efficient Multi-disciplinary team.

The unit is an integrated team of clinical staff and professionals including Nurses, Physiotherapists, Occupational Therapists and Social Workers,

The services provide care for patients requiring support and rehabilitation prior to discharge to their home.

Job description

Job responsibilities

  • Promote the goals of the unit to support appropriate referral into the service and plan effective discharge home with the MDT.
  • To be responsible for achieving and monitoring clinical quality standards, quality goals and patient feedback within the unit
  • To lead, liaise and communicate effectively with staff ensuring sharing of information and service development via regular team meetings and patient care plan reviews.
  • To support clinical management of highly complex cases which may include provision of appropriate care, patient assessment or review as required including communicating sensitive information to patients carers and staff.
  • Responsible for maintaining own clinical competency as per service specific training matrix
  • Undertake documentation audits within team to ensure compliance with Trust and individual professional standards for record keeping.


Person Specification

Qualification and Professional Training

Essential

  • Registered Nurse, Therapist, Social Worker educated to degree level (or equivalent)
  • Current Professional registration
  • Independent non-medical prescriber or willing to undertake
  • Teaching/Mentorship Qualification
  • Evidence of further recent study/continuous professional development
  • Improvement practitioner qualification or willing to work towards
  • Post Graduate Leadership Qualification or willing to work towards or relevant experience
  • Specialist practitioner qualification (degree / diploma) or equivalent level of professional qualification/ experience


Desirable

  • Educated to masters level or working towards/equivalent experience

  • Experienced in undertaking complex clinical or care-based interventions
  • Awareness of Workforce Policies and Procedures and experience of implementing these
