Hywel Dda Health Board are currently looking to recruit a Full-time Community Mental Health Nurse to join the Community Mental Health Team, based at Gorwelion Resource Centre, Aberystwyth.

Our services work through an assessment & recovery approach with people accessing secondary mental health services for severe and enduring mental illness.
The post holder will be responsible for the assessment of care needs, the development of programmes of care and the implementation and evaluation of these programmes of care for a defined clinical caseload including the role of a Care Coordinator within the Care Treatment Planning under the Mental Health (Wales) Measure 2010.
The post holder is required to work without direct supervision for their delegated caseload. The post holder will have effective communication skills and develop positive working relationships between all healthcare professionals, statutory and voluntary agencies. Maintaining good communication across all boundaries.
Main duties of the job
Gorwelion CMHT are looking for an enthusiastic, forward thinking individual who has a positive approach to management of change, and whose main duties will include:
To adopt a systematic approach to all patient care, taking responsibility for all stages of assessment, planning , implementation and evaluation of patient care., working explicitly within the requirements of the Welsh Mental Health Measure.
Management of a delegated caseload.
Participate as an active member of the CMHT and liaise with other agencies and teams as required
To recognize the involvement of carers and to provide and identify support for them as appropriate
To participate in the provision of Duty Officer.
The ability to speak Welsh is desirable for this post English and/or Welsh speakers are equally welcome to apply.
