We are a vibrant innovative Integrated Care Team, with a passion for providing excellent care for our patients. We work closely with GP's and allied Health Care Professionals and have a One Team Approach, embracing staff development and learning.
We are a busy happy team, who welcome new team members. We are seeking either newly qualified and or experienced nurses to provide complex care and treatment to adults living in the area who have long-term or complex health needs. You will work in a multi-disciplinary way alongside a range of specialist services, including those in the community, social and voluntary services. The team work closely with GPs and developing these relationships for the benefit of our patients is a key part of the role.

Main duties of the job
The post holder will have responsibility for managing, co-ordinating and delegating work ensuring referrals are dealt with according to clinical need, holistic assessments and care plans are recorded in line with Trust and Professional record keeping standards and clinical care at all times meets acceptable standards of patient safety and quality outcomes.
Working within Professional guidelines and accountability, the post-holder will have responsibility for the management of a defined community caseload (via Nursing, mental health, Physiotherapy or Occupational Therapy interventions) for a group of GP Practices as part of the Integrated Care Team.
The post holder will be required to work with a high degree of autonomy and professional accountability. A primary aim of the role is to prevent unnecessary admission of patients to an acute hospital and support timely discharge into the community. The post holder will also have a responsibility for re-enablement, rehabilitation, health protection and promotion programmes that improve and reduce inequalities.

The transformation will happen in stages Hampshire CAMHS, part of Sussex Partnership NHS Foundation Trust joined Southern Health in February 2024, with Isle of Wight NHS Trust mental health and community services transitioning across in May 2024. The final combination of Southern Health and Solent NHS Trust services is expected in late 2024, when our new organisation, to be known as Hampshire and Isle of Wight Healthcare NHS Foundation Trust, will be formed.
Our new Trust will continue to cater to the unique needs of different communities, making healthcare across the county more accessible, as well as offering staff more opportunities for career development, training and partnership working.
