An exciting opportunity has arisen for an experienced RNLD to work as part of a Community Learning Disabilities team in South Warwickshire.

The role involves close collaborative working with multi-disciplinary colleagues in the community setting, managing own caseload, Supervising others, completing assessments, raising awareness of the health needs of adults who have a learning disability, identifying unmet health needs and providing specialist nursing advice.

The successful candidate will need to be proactive, self-motivated and have experience of autonomous working, whilst also working under the direction of the community team manager.

The successful candidate will also be able to use current best practice to gain the best outcomes for Clients and their Carers. Will also assist in improving links with primary care services including hospitals/ G.P surgeries, provide support, information and education to clients, carers and provider's and also deliver education to people with learning disabilities around health needs both on 1:1 or group basis.

Main duties of the job

The post holder is responsible for the specialist assessment of client needs, including those with complex needs and/or behaviours that challenge, the development, implementation and evaluation of programmes of care. The post holder will have responsibility for a caseload. The post holder will also act as a health facilitator for people with a learning disability.

Job responsibilities

To promote the needs and rights of people with learning disabilities to other agencies and the general public.

To identify and evaluate specific client needs through specialist assessment.

To assist in the development of new services and projects within the community to meet identified clients needs, including those with complex needs and/or behaviours that challenge.

To participate as an active member of the Learning Disability Nursing Team and establish and maintain effective communication with other agencies and professionals.

To act as a health facilitator for people with a learning disability.

To report as soon as possible to the Team Manager any difficulties that may arise.

Essential

  • RNLD
  • Degree
  • Teaching qualification
  • CPD


Experience

Essential

  • RNLD with a substantial post registration experience at Band 5
  • Experience of working in multi agency teams with multi-disciplinary colleagues
  • Experience of working as a community nurse (PWLD)

Essential

  • Able to work well within multi agency teams and to demonstrate flexibility of approach.
  • Able to translate skills into a variety of settings / services and enthuse others.
  • Driving


Knowledge

Essential

  • Knowledge and understanding of up-to-date legislation in respect of Learning Disability Service
  • Demonstrate understanding of responsibilities of registered nurse according to NMC advisory codes
  • Knowledge and understanding of research and its relationship with practice.
  • Highly effective communication and liaison skills
  • Ability to manage own workload and to work in an autonomous capacity
  • Clinically proficient and able to provide direct assistance to clients with complex needs.
