We are excited to share an opportunity for a Band 6 Community Mental Health Nurse (multiple vacancies) .

This role involves working within one of our hub located in Bramdean and providing a single point of access for all emergency, urgent and routine referrals. As a Band 6 (NMC, HCPC OT, SWE) you will be working alongside both junior and senior practitioners. You will take a lead in coordinating shifts, delegating to other team members as well as liaising with other key stakeholders. You will be required to urgently respond to request for assessment and treatment of people experiencing a mental health crisis. You will work autonomously as well as an effective team member.

This is an integrated role in order to support the triage and/or rapid response as well as the Home Treatment elements of the service and the post holder will be responsible for creating an environment which is safe, supportive and an easily accessible with a no wrong door approach.

Qualified nurses (NMC) are currently benefiting from Retention Allowance. It is a 10% pay uplift on the top of 5% HCAS Fringe payment.This premium issubject to a minimum total payment of �3,553 and a maximum total payment of �4,528 p.a. This will be paid on a monthly basis and pro-rata'd to hours worked.

Main duties of the job

  • Adhere to the code of practice/conduct of your registering body at all times
  • Discharge professional responsibilities under the Mental Health Act (1983) and revised subsequent act Mental Capacity Act (2005) and the NMC Professional Code of Conduct
  • Be accountable and responsible for own practice.
  • Develop, implement and evaluate evidence based programmes of care set standards of care and liaise with other agencies, particularly in relation to people discharged under the Care Programme Approach.
  • Participate in the multi-disciplinary assessment of individuals with mental health problems or vulnerabilities living in the community, by providing a community mental health perspective and expertise

As this role is based within a community setting we do require that you must be able to travel between multiple sites and trust locations within the set hours of your working day.
