Meadow House Hospice is excited to announce the expansion of our palliative care service and we are looking for a dedicated nurse or paramedic, wishing to undertake a 12 month development programme to develop into a Band 7 Palliative Care Clinical Nurse Specialist or Palliative Care Paramedic.We are looking for someone who is passionate about delivering exceptional palliative care. They must be skilled in collaborative working with a variety of healthcare professionals and will be supported by senior staff in assessing and managing a caseload of patients. They must be committed to improving patient and carer experience and supporting our service expansion and development.

We support patients and families across the boroughs of Ealing and Hounslow working closely with our GP and D/N colleagues. We will provide and support you in achieving a core set of capabilities, under the supervision of experienced colleagues and once these are achieved there will be a guaranteed band 7 position.

Main duties of the job

To work under guidance visiting patients in their own homes, providing symptom control advice, psychological support, carer and financial support in conjunction with other members of the Meadow House Team.

Maintaining a shared caseload with a small group of CNS's. Keeping comprehensive documentation.

Liaising with staff within our 15 bedded hospice regarding admissions and attending MDT's.

Gaining confidence and competence in teaching, audit and research.

Person Specification

Education & Qualifications

Essential

  • Professional qualification, eg RN, HCPC
  • Degree in palliative care or equivalent specialist setting


Desirable

  • Extended prescribing certificate or willingness to obtain.
  • Advanced communication skills training
  • Level 2 psychology training
  • Level 7 post graduate qualification


Knowledge & Experience

Essential

  • Experience within a palliative care setting
  • Experience of identifying education needs
  • Experience of leading change
  • Knowledge of current palliative care agenda to include End of Life Care


Desirable

  • Experience of working within clinical governance and risk management systems


Skills, Abilities and Attributes

Essential

  • Evidence of a flexible approach to working independently as well as within an MDT structure
  • Time management/organisational skills and ability to work under pressure
  • Negotiation and facilitation skills
  • Excellent communication and inter-personal skills and ability to work under pressure
  • Practice development skills
  • Access to a car or other mode of transport for work purpose
  • Basic computer skills
