An opportunity has arisen for an enthusiastic and highly motivated nurse to join the Tissue Viability Team within the Dartford & Gravesham NHS Trust
We are seeking an individual who is interested and experienced in the care of patients with wounds. You will be reliable, organized, as well as being kind and compassionate. You will be able to work independently as well as being part of the team.
We are looking for someone with excellent communication skills. Someone who will demonstrate a passion and enthusiasm for teaching and educating staff at all levels, within our organisation. Part of your role will be to contribute to the ongoing implementation of the pressure ulcer prevention policy and the development of other tissue viability policies and guidelines, in conjunction with the Tissue Viability Nurse Specialists. You will be responsible for the assessment of patients with complex wounds/skin conditions and for the implementation of individualised treatment /management plans.

The following abilities /skills, attributes and experience are necessary for this post: o Experience of caring for patients with wounds o Excellent communication skills both written & verbal o Ability to teach, supervise and motivate staff o Relevant specialist qualification (e.g. tissue viability/wound care/leg ulcers) o To be a lead for staff regarding tissue viability issues. o To assist in the management of patients with complex wounds, o Act as a clinical role model and expert and ensure highest quality care is given.

Main Duties and Responsibilities:
Act as an effective role model for best practice (evidence-based), and acting as a specialist in tissue viability, giving clinical advice as necessary.
Utilise clinical assessment skills to examine, assess, treat and advise on the clinical management for patients with tissue viability needs and/or complex wounds.
To support health care professionals/staff in the delivery of quality, holistic and individualised plans of care with regard to tissue viability. Assist with complex dressing procedures in the clinical areas.
To provide support for vascular patients, offering specialist advice on the identification and management of leg ulcers/vascular skin conditions.
To participate in new initiatives, which will lead to demonstrable improvements in the documentation of care, in relation to tissue viability.
To ensure that all patients/carers and staff have access to relevant information regarding the prevention and management of pressure ulcers and of wound care treatments utilised.
To participate in the development of Trust-wide projects pertinent to tissue viability e.g. Leg Ulcer Pathways, Pressure Ulcer Pathway. Interpretation and implementation of National Guidelines, ensuring best practice is adopted across the organisation.
To adhere to the NMC Code of Professional Conduct and be wholly accountable for his/her practice.
To exercise professional skills in a manner congruent to the NMCs position on The Scope of Professional Practice.
Actively protect the rights and enhance the integrity of individuals and groups.
