Do you have a passion for medicines optimisation and pharmaceutical care in a specialist service? Do you want to support the development of the new pharmacy workforce? This post is a well-established collaboration between UHBW Pharmacy Department and University of Bath with an exciting opportunity for a Second Trust Practice Educator post to support the expanding clinical-facing component of the Undergraduate degree programme.

We are looking for an enthusiastic and motivated pharmacist to lead patient flow and medicines optimisation across our cardiology service including providing specialist pharmacist input to the newly funded mavacamten MDT, supporting patients across the South West from our Bristol Heart Institute. The post-holder will work alongside a team of pharmacy professionals working in this area:

Our cardiac pharmacy team:
  • Clinical Pharmacy Manager
  • Lead Cardiac Pharmacist
  • Specialist and Foundation Rotational Pharmacists
  • Specialist Pharmacy Technician
  • Alongside close collaboration with our Anticoagulation and Thrombosis Pharmacy Team

The postholder will work collaboratively with the Lead Pharmacist for Education and Training and the other UHBW Practice Educator Pharmacist to design and deliver undergraduate teaching at UHBW, with particular focus on the successful Clinical Learning in Practice model.

Main duties of the job

  • Working as a specialist cardiac pharmacist on our tertiary centre cardiology wards, prioritising patients to complete medicines reconciliation, managing interface issues, providing specialist input and optimising medicines for acutely unwell patients, and on discharge to ensure appropriate review and management of cardiac medications in the community
  • Acting as pharmacist link to the monthly mavacamten MDT, advising on appropriate dosing, interpretation of genomic results, completing patient consultations and advising on issues such as drug interactions, bioavailability considerations and stock management issues. Supporting the delivery of an appropriate mavacamten service across the geography through a hub (UHBW) and spoke (other Trusts) model
  • Designing and delivering high quality teaching which is informed by significant professional experience gained via practice, research, study and teaching experience to undergraduate students on the new MPharm programme.
  • 0.4 WTE of the role will be patient-facing, leading all aspects of the clinical pharmacy service to our cardiology wards, 0.2 WTE will focus on specialist pharmacist input to our mavacamten patient cohort and MDT, and the other 0.4 WTE will be focusing on designing and delivering a whole range of undergraduate pharmacy teaching, with the principle being delivery of teaching on-site at UHBW

As a member of the Pharmacy team you will also participate in:
  • Our late duty rota in our BRI dispensary, as overtime
  • Our Saturday & Sunday shifts, as part of normal contracted hours with commensurate 'off-duty days', on a rota basis of roughly one weekend day every 6 weeks
  • Our Bank Holiday rota, typically include 2 shifts per annum with commensurate lieu time.


Person Specification

Qualifications and Training

Essential

  • Master's degree (or equivalent) in pharmacy.
  • Registration with General Pharmaceutical Council (GPhC).
  • Evidence of continuing professional development (CPD).
  • Postgraduate Diploma or MSc in Clinical Pharmacy


Desirable

  • Membership of a recognised clinical pharmacy professional body
  • Qualification in Independent Prescribing - NB: To be undertaken on appointment if not already a registered non-medical prescriber)
  • Teaching Qualification - (N.B. support and development will be provided by the University)


Knowledge and Experience

Essential

  • A thorough knowledge of UK hospital pharmacy practice, safe management at transfers of care and medication commissioning
  • Medicines reconciliation in an acute care setting
  • Experience of one to one and small group teaching to a range of healthcare professionals of different levels of experience
  • Previous experience of audit & quality improvement
  • Ability to initiate, lead, manage and supervise a range of projects
  • Experience of managing and motivating staff, appraising and giving feedback
  • Knowledge of basic medicines information and critical appraisal techniques to apply medical / pharmacotherapeutic evidence to patient care
  • Knowledge of the legal framework for the dispensing, prescribing and administration of medicines
  • Knowledge of budgetary management and financial control
  • Understanding of medicine and nursing practice, record keeping and terminology

  • Ability to holistically & critically analyse prescriptions and treatment plans for clinical appropriateness, clinical risk, cost-effectiveness, adherence to local & national guidance and legality
  • Ability to use professional judgement to challenge inappropriate prescribing and otherwise influence prescribing decisions to enable optimal therapy
