Tissue Viability Nurse
An exciting opportunity to join the Tissue Viability team has arisen as a Band 6 nurse in this specialist service. The successful candidate will have a minimum of three years post registration experience, previous experience in tissue viability and a good understanding of pressure ulcer prevention policies and pathways. The post will include working across all ELHT and Blackburn with Darwen hospital sites and within the community, to provide expert assessment and treatment planning for patients with complex wounds. The successful candidate will have good communication skills and work well with professionals from a range of disciplines.
Main duties of the job
o Under the guidance of the TVNS and in conjunction with the multi-disciplinary Teams provide a high standard of skilled care in the field of tissue viability within all Trust settings.o Work within the trust and national policies and standardso To provide specialist knowledge and advice to colleagues and patients in the multi-disciplinary environmento To provide high level clinical skill when giving direct care to patientso Using evidence based practice assess, implement and evaluate programmes of care to meet the needs of patientso To adopt a holistic approach to patients / carers and familyo Liaise with nursing and allied health professionals to create good working relationships for the benefit of patients.o To monitor the patient's condition and report any significant changes or concerns to the TVNS or other professionals as appropriate.o To ensure nursing interventions are clinically effective and evidence based.
o Teach and support staff in the trust in relation to all aspects of tissue viability

Promote peoples equality, diversity and rights Continually monitor standards of care and assist in the improvement, participate in benchmarking, audit and research Record activity and input data to allow reports to be generated to demonstrate the activity and outcomes within tissue viability Maintain data base of clinical incidents relating to pressure ulcers, identify trends and training needs in partnership with TVNS The post holder will identify and highlight to TVNS opportunities for improvements in nursing practice, service development and quality initiatives Attend and contribute at staff meetings and working groups to identify areas of improvement to ensure targets and objectives are met. Be familiar and comply with clinical governance initiatives in practice. The post holder will be a member of appropriate committees and groups supporting the TVNS Initiate and participate in audit and evaluations, complying with trust guidance.
