Southampton Urgent Response Service are looking for a Registered Occupational Therapist to join our dynamic multidisciplinary team.
The Urgent Response Service is a supportive, fast paced, interdisciplinary community team aimed at preventing unnecessary admissions to hospital and facilitating timely discharges from hospital. It is an integrated health and social care team that also visits patients in the Emergency Department to facilitate rapid discharge. Patients are over 18 with a wide range of health and social care needs.
The service provides assessment, clinical monitoring and therapy input to promote recovery. The team promotes interprofessional working which supports the single assessment process for patients. There is a robust supervision frame and opportunities for training and development. You will need effective communication and interpersonal skills, be flexible and have a positive approach to problem solving.
Having a valid driving license and access to a car is essential.
The staff member will be required to work over a seven-day period, currently working 1 in 4 weekends.

We have been working with other local NHS Trusts to create a new organisation, bringing together all community, mental health and learning disability services from across Hampshire and Isle of Wight. Subject to approvals, the aim is for the new Trust to be formed by the end of 2024.
Project Fusion is the name for the programme to create the new combined NHS Foundation Trust. Hampshire and Isle of Wight Healthcare NHS Foundation Trust will be bringing the services into a single organisation will result in more consistent care, more equitable access to services irrespective of postcode, and a more sustainable workforce and services. The new organisation will operate locally to ensure services can best meet the needs of different communities.
The new Trust will be comprised of all the services currently provided by Solent NHS Trust and Southern Health NHS Foundation Trust, the community, mental health and learning disability services provided by Isle of Wight NHS Trust and child and adolescent mental health services delivered in parts of Hampshire by Sussex Partnership NHS Foundation Trust.
Colleagues will transfer to the new organisation under Transfer of Undertakings (Protection of Employment) -- known as TUPE on the day the trust is formed. TUPE is a legal process to ensure you transfer out of one organisation and into another, maintaining the terms and conditions of your employment.

Main duties of the job
- To work as part of an integrated health and social care team providing a crisis intervention and early supported discharge service for patients across the City of Southampton registered with a Southampton CCG GP practice.
- To manage a caseload with supervision from senior staff
- Undertake assessment and treatment of patients within a home setting and determine intervention in order to promote recovery from illness and rehabilitation, guided by more senior clinicians as necessary.
- To work collaboratively with partners in health and social care in order to avoid unnecessary hospital admission and facilitate early and safe discharge from hospital following an acute episode of care.
