You will work across the transition's pathway, working primarily with young people 17 years and upwards. This is to ensure that young adults are supported through the pathways between children and adult services. You will work as part of a team in delivering care for younger adults who have a wide range of complex mental health needs. To develop your role, you will be supported with an induction package, both local and corporate and you will receive regular supervision and career conversations annually. You will be managed by a senior clinician in the team providing a structured clinical model to the client group.
It is widely recognised that the transition point at the age of eighteen is a challenging one for young adults but particularly if they have mental health difficulties. Somerset NHS Foundation Trust is committed to the development of an improved transition pathway, with the aim of ensuring a smooth transition between services as needed.
This position is for a countywide post and we will accept applicant who are registered with either the NMC or HCPC.
Main duties of the job
You will need to work directly with young people in Somerset with complex emotional needs transitioning from CAMHS to adult services. The people we work with often have extensive trauma histories and have experienced multiple ACEs. Therefore we are looking to recruit someone who can work relationally and in a trauma informed way, while at the same time maintaining professional boundaries. The people we work with can often present with a variety of risks, such as severe self harming behaviour. The post holder will need to have confidence in managing this and be able to use support and supervision appropriately.

Pillars of health working on good sleep, good food and fluid intake, environmental stability, reducing drug and alcohol use, learning to live with pain or physical illness, thinking about medication as needed
Reconnection getting back to life. Education, work, volunteering, socialising, hobbies/activities, beginning/maintaining/ending relationships with others.
This is a countywide position, so the post holder will need to have the ability and willingness to travel.
You will be responsible for their own CPD as well as having some supervisory responsibility for junior members of the team.
The main therapeutic model in Connect 18 is MBT. We aim to provide you training in this.

Job Purpose:
We are looking to recruit a highly motivated individual to work with us to improve the transition across services for young people.
We are looking for a Band 5 practitioner to join Connect18 based countywide. We are looking for a dedicated and passionate individual who wants to develop their professional skills and gain experience in working in community with young people transitioning from CAMHs and with complex mental health, social, emotional, behavioural, and neurodevelopmental needs.
This post is open to professionals registered with the relevant professional body (NMC/HCPC) and experience of working with young people. This is a developmental role with a focus on developing broader skills in Mentalisation Behaviour Therapy, attachment theory, trauma recovery model, and positive risk taking and behavioural management. The role offers opportunities for you to develop your clinical skills and knowledge.
We will ensure you have all the core skills you need to confidently build relationships with our service users in order to best create and deliver an effective and personalised treatment plan with them. This is an exciting opportunity for anyone wanting to be part of a growing and dynamic transitions service.
You will demonstrate a desire to engage in and promote a wide range of therapeutic activities to support service users. Individuals will hold qualities of patience, enthusiasm and a keen interest in working with young people with complex emotional needs.

Connect18 has a phased care model which is trauma informed, with a strengths-based approach and practical support alongside. The role involves working under the direction of senior clinicians within the team, but also being accountable for your own practice, and using one's own initiative when appropriate to support smooth transition for young adults.
The successful candidate must have a specific interest and experience in working with young adults, and their people who matter.
Duties and Responsibilities
Communication and Key Working Relationships
Exceptional interpersonal skills with the ability to communicate effectively with staff, patients, carers and relatives (as appropriate) remaining sensitive and empathetic.

Act as the patient's advocate where necessary working in partnership with other health care professionals, agencies, and independent advocacy.
